Description
Overview of the medicine
Abiraterone Acetate is an antiandrogen medication used for the treatment of prostate cancer. It inhibits the enzyme CYP17A1, which is crucial for androgen biosynthesis.
Uses & Indications
Dosage
Adults
1000 mg (four 250 mg tablets) orally once daily. Must be taken on an empty stomach (at least 1 hour before or 2 hours after food). Administer in combination with 5 mg prednisone or prednisolone orally twice daily.
Elderly
No specific dose adjustment needed.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. Caution with severe renal impairment, monitor for toxicity.
How to Take
Oral. Tablets should be swallowed whole with water, not chewed or crushed. Take on an empty stomach (at least 1 hour before or 2 hours after food). Administer with prednisone or prednisolone.
Mechanism of Action
Abiraterone is a prodrug converted in vivo to abiraterone, an androgen biosynthesis inhibitor. It selectively inhibits the enzyme CYP17A1 (17α-hydroxylase/C17,20-lyase), preventing the production of androgens like testosterone and dihydrotestosterone, which fuel prostate cancer cell growth.
Pharmacokinetics
Onset
Maximum plasma concentrations typically reached within 2 hours. Therapeutic effect observed over weeks.
Excretion
Primarily via feces (89%), with a smaller amount excreted in urine (5%).
Half life
Approximately 12-15 hours.
Absorption
Oral absorption is variable and increased by food (especially high-fat meals). Must be administered on an empty stomach.
Metabolism
Primarily hepatic, via CYP3A4 and SULT2A1, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to abiraterone acetate or any excipients.
- Pregnancy (can cause fetal harm).
- Severe hepatic impairment (Child-Pugh Class C).
Drug Interactions
Drugs Causing QT Prolongation
Use with caution, monitor ECG.
Strong CYP3A4 Inducers (e.g., Rifampin, Phenytoin)
May decrease abiraterone exposure, reduce efficacy. Avoid coadministration.
Drugs Metabolized by CYP2D6 (e.g., Metoprolol, Thioridazine)
Abiraterone can inhibit CYP2D6, increasing exposure to these drugs. Monitor patients.
Storage
Store below 30°C in a dry place, away from light. Keep out of reach of children.
Overdose
No specific antidote. Management involves general supportive care and monitoring for signs of mineralocorticoid excess (e.g., hypertension, hypokalemia, fluid retention) and hepatotoxicity.
Pregnancy & Lactation
Contraindicated in women who are or may become pregnant due to potential fetal harm. Not indicated for women; unknown if excreted in human milk.

Clinical Trials
Ongoing clinical trials explore its use in earlier stages of prostate cancer and in combination with other agents.
Lab Monitoring
- Liver function tests (ALT, AST, bilirubin) before treatment, every two weeks for the first three months, and monthly thereafter.
- Serum potassium, blood pressure, and fluid retention regularly.
Doctor Notes
- Emphasize strict adherence to empty stomach administration.
- Co-administer with prednisone/prednisolone is crucial to mitigate mineralocorticoid excess.
- Close monitoring of liver function and electrolytes is essential.
Patient Guidelines
- Take exactly as prescribed with prednisone/prednisolone.
- Take on an empty stomach: at least 1 hour before or 2 hours after food.
- Do not chew or crush tablets.
- Report any signs of liver problems (e.g., yellowing skin/eyes, dark urine, severe nausea), severe fatigue, or symptoms of mineralocorticoid excess (e.g., swelling, leg weakness).
Missed Dose Advice
If a dose is missed, take the next dose at the regularly scheduled time. Do not take a double dose to make up for a missed one.
Driving Precautions
Abiraterone is not expected to affect the ability to drive or operate machinery. However, if experiencing fatigue or dizziness, advise caution.
