Aceclofenac 100 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis. It works by reducing the production of certain natural substances in the body that cause inflammation and pain.
Uses & Indications
Dosage
Adults
The recommended dose is 100 mg twice daily, one tablet in the morning and one in the evening.
Elderly
No dosage adjustment is generally required, but caution is advised, and the lowest effective dose should be used.
Renal_impairment
No evidence suggests the need for dose modification in patients with mild renal impairment. Caution is advised in moderate to severe renal impairment.
How to Take
Take orally, swallow the tablet whole with a glass of water, with or after food.
Mechanism of Action
Aceclofenac primarily acts as an inhibitor of prostaglandin synthesis. It selectively inhibits cyclooxygenase-2 (COX-2), the enzyme responsible for the synthesis of prostaglandins involved in inflammation, pain, and fever. It also exhibits some inhibition of COX-1, which is involved in maintaining gastric mucosal integrity.

Excretion
Approximately two-thirds of the administered dose is excreted in the urine, mainly as hydroxylated metabolites, and one-third via the faeces.

Absorption
Rapidly and completely absorbed orally, peak plasma concentrations reached in 1.25 to 3 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to 4'-hydroxyaceclofenac and other metabolites.

- Hypersensitivity to aceclofenac or any excipients
- Patients with active gastrointestinal bleeding or peptic ulcer
- History of NSAID-induced asthma, urticaria, or allergic-type reactions
- Severe heart failure, renal failure, or hepatic failure
- Third trimester of pregnancy

Lithium
Increased plasma lithium levels.
Warfarin
Increased risk of bleeding.
Methotrexate
Increased methotrexate toxicity.
Diuretics and ACE Inhibitors
Reduced hypotensive effect and increased risk of renal impairment.
