Description
Overview of the medicine
Aceclofenac is a non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain and inflammation in conditions such as osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis. It works by reducing the production of prostaglandins, which are substances in the body that cause pain and inflammation.
Uses & Indications
Dosage
Adults
The usual recommended dose is 100 mg twice daily (one tablet in the morning and one in the evening).
Elderly
No dosage adjustment is generally required, but caution is advised due to increased risk of side effects. Start with the lowest effective dose.
Renal_impairment
No specific dose reduction is usually required for mild renal impairment, but caution is advised. Avoid in severe renal impairment.
Hepatic_impairment
The initial dose should be reduced to 100 mg once daily in patients with mild to moderate hepatic impairment.
How to Take
Take the tablet whole with a sufficient amount of water, preferably with or after food, to minimize gastrointestinal discomfort. Do not crush or chew.
Mechanism of Action
Aceclofenac primarily acts by inhibiting the enzyme cyclooxygenase (COX), which is involved in the synthesis of prostaglandins. It shows a preferential inhibition of COX-2 over COX-1, leading to reduced inflammation and pain with potentially fewer gastrointestinal side effects compared to non-selective NSAIDs.

Onset
Pain relief may start within 30-60 minutes.
Excretion
Approximately two-thirds of the administered dose is excreted in the urine, mainly as hydroxylated metabolites, and about one-third in the faeces.
Half life
Approximately 4-4.3 hours.
Absorption
Rapidly and completely absorbed after oral administration, peak plasma concentrations reached in 1.25 to 3 hours. Bioavailability is approximately 98%.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to 4'-hydroxyaceclofenac as the main metabolite.
Side Effects
Contraindications

- •History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs

Drug Interactions
Digoxin
Increased plasma digoxin levels.
Lithium
Increased plasma lithium levels.
Diuretics
Reduced diuretic effect and increased risk of nephrotoxicity.
Methotrexate
Increased plasma methotrexate levels and toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Quinolone Antibiotics
Increased risk of convulsions.
Cyclosporine, Tacrolimus
Increased risk of nephrotoxicity.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store in a cool, dry place, away from direct light and moisture. Keep out of reach of children. Do not store above 30°C.
Overdose
Symptoms of overdose may include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, rarely diarrhea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, fainting or convulsions. In cases of significant poisoning, acute renal failure and liver damage are possible. Treatment is primarily symptomatic and supportive; gastric lavage or activated charcoal may be considered within an hour of ingestion.
Pregnancy & Lactation
Pregnancy Category C (first and second trimesters); avoid in third trimester due to potential premature closure of ductus arteriosus and impaired renal function. Aceclofenac is excreted in breast milk in small amounts, so use with caution in breastfeeding mothers.
