Overview of the medicine
Acenac 100 mg Tablet contains Aceclofenac, a non-steroidal anti-inflammatory drug (NSAID) used for pain and inflammation. It provides relief from conditions such as osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis.
Uses & Indications
Dosage
Adults
The recommended dose is 100 mg twice daily, preferably with food.
Elderly
Lower doses may be considered, usually 100 mg once daily, with careful monitoring. Dose adjustment is generally not required unless renal or hepatic impairment is present.
Renal_impairment
Initiate with caution, usually 100 mg once daily. Avoid in severe renal impairment (creatinine clearance < 30 mL/min).
How to Take
Swallow the tablet whole with water, preferably during or after a meal, to minimize gastrointestinal discomfort. Do not chew or crush the tablet.
Mechanism of Action
Aceclofenac primarily inhibits prostaglandin synthesis by inhibiting cyclooxygenase (COX) enzymes, particularly COX-2, which reduces inflammation, pain, and fever. It also has some effects on COX-1.

Excretion
Approximately two-thirds of the administered dose is excreted in the urine and one-third in the faeces, mainly as hydroxylated metabolites.

Rapidly and completely absorbed from the gastrointestinal tract following oral administration, reaching peak plasma concentrations within 1.25 to 3 hours.
Metabolism
Extensively metabolized in the liver, primarily by cytochrome P450 2C9, to its main metabolite, 4'-hydroxyaceclofenac.

- Hypersensitivity to aceclofenac, aspirin, or other NSAIDs

- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs

Symptoms of overdose may include nausea, vomiting, epigastric pain, gastrointestinal bleeding, and rarely, diarrhea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, and convulsions. In cases of significant poisoning, acute renal failure and liver damage are possible. Management is primarily supportive and symptomatic; gastric lavage and activated charcoal may be considered within an hour of ingestion.
Pregnancy & Lactation
Avoid during pregnancy, especially in the third trimester, due to potential fetal harm (e.g., premature closure of ductus arteriosus, renal dysfunction). Use with caution during lactation; small amounts may pass into breast milk, but adverse effects on the infant are unlikely with therapeutic doses.
