Overview of the medicine
Aceclofenac is a non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis.
Uses & Indications
Dosage
Adults
The recommended dose is 100 mg twice daily, one tablet in the morning and one in the evening. It should be taken with or after food.
Elderly
Generally, no dose adjustment is required, but caution is advised, and the lowest effective dose should be used. Renal and hepatic function should be monitored.
Renal_impairment
In mild to moderate renal impairment, no dosage adjustment is usually necessary, but caution is advised. Avoid in severe renal impairment.
How to Take
Acflam 100 mg tablet should be swallowed whole with a sufficient quantity of liquid. It can be taken with or after food to minimize gastrointestinal discomfort.
Mechanism of Action
Aceclofenac acts primarily as a selective COX-2 inhibitor, reducing the synthesis of prostaglandins, which are key mediators of inflammation, pain, and fever. It also has some inhibitory effect on COX-1.
Pharmacokinetics
Onset
Onset of analgesic action typically within 30-60 minutes.
Excretion
Approximately two-thirds of the administered dose is excreted in the urine, and one-third in the faeces.
Half life
The plasma elimination half-life is approximately 4 hours (range 4-4.3 hours).
Absorption
Rapidly and completely absorbed after oral administration. Peak plasma concentrations are reached approximately 1.25 to 3 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C9, to its main metabolite, 4'-hydroxyaceclofenac.
Side Effects
Contraindications
- •Hypersensitivity to aceclofenac, aspirin, or other NSAIDs (e.g., patients who have experienced asthma, acute rhinitis, angioedema, or urticaria after taking aspirin or other NSAIDs)
- •Active or recurrent peptic ulcer/hemorrhage
- •Severe heart failure (NYHA Class II-IV)
- •Severe renal impairment
- •Severe hepatic impairment
- •Third trimester of pregnancy
Drug Interactions
Lithium
Increased plasma lithium levels, leading to potential toxicity.
Methotrexate
Increased methotrexate toxicity due to reduced renal clearance.
Corticosteroids
Increased risk of gastrointestinal ulceration or bleeding.
Diuretics and ACE inhibitors
Reduced antihypertensive and diuretic effects.
Warfarin and other anticoagulants
Increased risk of bleeding due to enhanced anticoagulant effect.
SSRIs (Selective Serotonin Reuptake Inhibitors)
Increased risk of gastrointestinal bleeding.

Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, rarely diarrhea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, and convulsions. Management is symptomatic and supportive; gastric lavage, activated charcoal, and forced diuresis may be considered.
Pregnancy & Lactation
Pregnancy: Contraindicated in the third trimester due to risk of premature closure of the fetal ductus arteriosus. Use in the first and second trimesters only if the potential benefit outweighs the potential risk to the fetus, and at the lowest effective dose for the shortest duration. Lactation: Avoid during breastfeeding as aceclofenac is excreted in human milk.
