Description
Overview of the medicine
Achil 200 mg Tablet is a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation associated with various conditions such as osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis. It works by selectively inhibiting the COX-2 enzyme, which is involved in prostaglandin synthesis.
Uses & Indications
Dosage
Adults
Oral, 200 mg once daily, or 100 mg twice daily. Maximum daily dose 200 mg.
Elderly
No specific dose adjustment is usually required, but caution is advised due to increased risk of side effects. Consider lower starting dose.
Renal_impairment
Mild to moderate renal impairment: No dose adjustment. Severe renal impairment (CrCl < 30 mL/min): Not recommended.
How to Take
Take the tablet orally with water, preferably with food or after meals to minimize gastrointestinal discomfort. Do not crush, chew, or break the tablet.
Mechanism of Action
Achil selectively inhibits the cyclooxygenase-2 (COX-2) enzyme, which is responsible for the synthesis of prostaglandins, inflammatory mediators. By reducing prostaglandin production, it diminishes inflammation, pain, and fever.
Pharmacokinetics
Onset
Pain relief typically begins within 30-60 minutes.
Excretion
Excreted mainly through urine (60%) and feces (40%) as metabolites.
Half life
Approximately 8-10 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached within 2-3 hours.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes (CYP2C9 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to Achil or any NSAID (including aspirin)
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
- Active gastrointestinal bleeding or peptic ulcer
- Severe heart failure
- Severe renal or hepatic impairment
- Third trimester of pregnancy
Drug Interactions
Lithium
Increased plasma lithium levels, leading to toxicity.
Methotrexate
Increased methotrexate toxicity.
Diuretics and ACE inhibitors
Reduced antihypertensive effect and increased risk of renal impairment.
Warfarin and other anticoagulants
Increased risk of bleeding.
SSRIs (Selective Serotonin Reuptake Inhibitors)
Increased risk of gastrointestinal bleeding.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management involves symptomatic and supportive care. Gastric lavage or activated charcoal may be considered within an hour of ingestion.
Pregnancy & Lactation
Pregnancy Category C (first and second trimesters); Category D (third trimester). Avoid in the third trimester due to potential premature closure of the fetal ductus arteriosus. Use with caution during lactation; consult a doctor.
