Description
Overview of the medicine
Acical 500 mg Tablet is a calcium supplement used to prevent or treat low blood calcium levels in people who do not get enough calcium from their diets. It is essential for strong bones, healthy nerves, muscles, and a healthy heart. It can be used to treat conditions caused by low calcium levels such as osteoporosis, osteomalacia/rickets, hypoparathyroidism, and latent tetany.
Uses & Indications
Dosage
Adults
Typically 500 mg to 1200 mg elemental calcium per day, divided into 1 to 3 doses. For osteoporosis, 500-1000 mg daily. Acical 500 mg: 1-2 tablets daily.
Elderly
Same as adult dosage, but often higher doses are required due to decreased absorption. Consult physician.
Renal_impairment
Use with caution and monitor calcium levels. Dosage adjustment may be necessary to avoid hypercalcemia.
How to Take
Take the tablet orally with water, preferably with meals to enhance absorption and minimize gastrointestinal upset. Do not crush or chew sustained-release tablets if applicable.
Mechanism of Action
Calcium is a vital mineral that plays a crucial role in bone formation and maintenance, muscle contraction, nerve transmission, and various metabolic functions. As calcium carbonate, it provides a readily available source of calcium ions (Ca2+) which are absorbed from the gastrointestinal tract and incorporated into the body's calcium pool.
Pharmacokinetics
Onset
Effects on serum calcium levels are relatively rapid, typically within 1-2 hours after administration.
Excretion
Unabsorbed calcium is excreted in feces. Absorbed calcium is primarily excreted via the kidneys (glomerular filtration and tubular reabsorption) and to a lesser extent via bile and sweat.
Half life
The half-life of calcium in the bloodstream is complex due to its constant exchange with bone, but the elimination half-life from plasma is generally short, within hours.
Absorption
Approximately 20-30% of ingested calcium carbonate is absorbed, primarily in the duodenum and proximal jejunum, through active and passive transport mechanisms. Absorption is enhanced by Vitamin D.
Metabolism
Calcium itself is not metabolized in the traditional sense; it is a mineral. It's incorporated into bones or maintained in the free ion form.
Side Effects
Contraindications
- •Hypercalcemia (high blood calcium levels)
- •Severe hypercalciuria (high urinary calcium levels)
- •Renal calculi (kidney stones)
- •Hypersensitivity to calcium carbonate or any component of the formulation
Drug Interactions
Bisphosphonates
Calcium can reduce the absorption of bisphosphonates (e.g., alendronate). Administer calcium at least 30-60 minutes after bisphosphonates.
Thyroid hormones
Calcium can reduce the absorption of levothyroxine. Administer calcium at least 4 hours after thyroid hormones.
Thiazide diuretics
Can decrease urinary calcium excretion, potentially leading to hypercalcemia.
Tetracyclines & Quinolones
Calcium can chelate these antibiotics, reducing their absorption. Administer calcium at least 2-4 hours before or after these antibiotics.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcemia (nausea, vomiting, constipation, polyuria, polydipsia, muscle weakness, confusion, fatigue). Severe cases can lead to renal impairment and cardiac arrhythmias. Treatment involves discontinuing calcium, increasing fluid intake, and managing symptoms. In severe cases, intravenous fluids and diuretics may be required.
Pregnancy & Lactation
Calcium supplements are generally considered safe and often recommended during pregnancy and lactation to meet increased calcium requirements. Consult a healthcare provider for appropriate dosage.
