Description
Overview of the medicine
Acidron 4 mg Injection contains Ondansetron, a potent antiemetic used for the prevention and treatment of nausea and vomiting induced by chemotherapy, radiotherapy, or surgery.
Uses & Indications
Dosage
Adults
For chemotherapy-induced nausea and vomiting: 8 mg IV over 15 minutes immediately before chemotherapy. For postoperative nausea and vomiting: 4 mg IV slowly or IM at induction of anesthesia.
Elderly
No dose adjustment required for patients aged 65-74 years. For patients aged ≥75 years, the initial dose should not exceed 8 mg given over 15 minutes.
Renal_impairment
No dose adjustment is generally required for patients with renal impairment.
How to Take
Administer by slow intravenous (IV) injection over at least 30 seconds, preferably 2-5 minutes, or by intramuscular (IM) injection. For IV infusion, dilute the contents of the ampoule and administer over 15 minutes.
Mechanism of Action
Ondansetron is a selective serotonin 5-HT3 receptor antagonist. Serotonin 5-HT3 receptors are present both peripherally on vagal nerve terminals and centrally in the chemoreceptor trigger zone (CTZ) of the brain. By blocking these receptors, ondansetron prevents the initiation of the vomiting reflex.

Excretion
Approximately 5% of the dose is excreted unchanged in urine, with the remainder excreted as metabolites via urine and feces.
Half life
Approximately 3 hours (adults), 6-7 hours (elderly)
Absorption
Rapidly and completely absorbed after oral administration; bioavailability is about 50-60% due to first-pass metabolism. IV administration bypasses this.
Metabolism
Extensively metabolized in the liver, primarily by hydroxylation followed by glucuronide or sulfate conjugation.
Side Effects
Contraindications
- Hypersensitivity to Ondansetron or any component of the formulation.
- Concomitant use with apomorphine.
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol.
Apomorphine
Concomitant use with apomorphine is contraindicated due to reports of profound hypotension and loss of consciousness.
SSRIs/SNRIs
Caution advised as there is a theoretical risk of serotonin syndrome with concomitant use.
Phenytoin, Carbamazepine, Rifampicin
These potent CYP3A4 inducers may reduce plasma concentrations of ondansetron.
Storage
Store below 30°C. Protect from light. Do not freeze.

Symptoms of overdose include transient visual disturbances, severe constipation, hypotension, and vasovagal episodes with transient AV block. Treatment is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. Ondansetron is excreted in the milk of lactating rats; it is not known whether it is excreted in human milk. Exercise caution when administering to a nursing mother.
