Description
Overview of the medicine
Acubis 25 mg Tablet contains Losartan Potassium, an Angiotensin II Receptor Blocker (ARB), used to treat high blood pressure (hypertension) and to reduce the risk of stroke in patients with high blood pressure and an enlarged heart. It is also used to treat kidney disease in patients with type 2 diabetes and a history of high blood pressure.
Uses & Indications
Dosage
Adults
Hypertension: Initial 50 mg once daily, adjusted based on blood pressure response. Max 100 mg once daily. Stroke risk reduction: Initial 50 mg once daily. Diabetic nephropathy: Initial 50 mg once daily, may be increased to 100 mg once daily.
Elderly
No initial dosage adjustment is typically required, but patients over 75 years may start with a lower dose (e.g., 25 mg once daily) under medical supervision.
Renal_impairment
No initial dosage adjustment is generally required for mild to moderate renal impairment. For severe renal impairment or dialysis patients, a lower starting dose (e.g., 25 mg once daily) may be considered.
How to Take
Acubis 25 mg Tablet can be taken with or without food, usually once daily. It should be swallowed whole with water.
Mechanism of Action
Losartan Potassium selectively blocks the binding of angiotensin II to the AT1 receptor, which is found in many tissues (e.g., vascular smooth muscle, adrenal gland). This blockade inhibits the vasoconstrictive and aldosterone-secreting effects of angiotensin II, leading to vasodilation, decreased aldosterone secretion, and reduced blood pressure.

Absorption
Well absorbed from the gastrointestinal tract; undergoes significant first-pass metabolism. Bioavailability is about 33%.
Metabolism
Hepatic (CYP2C9 and CYP3A4) to an active carboxylic acid metabolite (E-3174), which is more potent than losartan.
Side Effects
Contraindications
- Hypersensitivity to losartan or any component of the formulation
- Pregnancy (especially second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes or renal impairment (GFR <60 mL/min/1.73 m2)
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
Diuretics and other antihypertensives
Additive hypotensive effects
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect and increase the risk of renal dysfunction, especially in dehydrated or elderly patients.
Potassium-sparing diuretics (e.g., spironolactone, triamterene, amiloride), potassium supplements, or salt substitutes containing potassium
May lead to hyperkalemia

Store at room temperature (below 30°C), away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypotension and tachycardia; bradycardia could occur from parasympathetic stimulation. Treatment is symptomatic and supportive. If ingestion is recent, induce emesis or perform gastric lavage. Administer activated charcoal. Losartan and its active metabolite are not removed by hemodialysis.
Pregnancy & Lactation
Contraindicated in pregnancy, especially in the second and third trimesters, due to risk of fetal injury and death. Not recommended during breastfeeding as it is unknown if losartan is excreted in human milk.
