Description
Overview of the medicine
Adalimumab is a tumor necrosis factor (TNF) blocker used to treat various autoimmune diseases by reducing inflammation and immune responses.
Uses & Indications
Dosage
Adults
Rheumatoid Arthritis, Psoriatic Arthritis, Ankylosing Spondylitis: 40 mg every other week. Crohn's Disease, Ulcerative Colitis: Initial dose 160 mg (Day 1) followed by 80 mg (Day 15), then 40 mg every other week.
Elderly
No specific dose adjustment required based on age; however, monitor for increased incidence of infections.
Renal_impairment
No specific dose adjustment required for patients with renal impairment.
How to Take
Administer by subcutaneous injection into the thigh or abdomen. Patients can be trained to self-inject.
Mechanism of Action
Adalimumab binds specifically to TNF-alpha and blocks its interaction with cell surface TNF receptors, thereby inhibiting pro-inflammatory cytokine activity and downstream inflammatory processes.
Pharmacokinetics
Onset
Clinical response typically observed within weeks to months, depending on the indication.
Excretion
Elimination occurs through the reticuloendothelial system; no specific excretion pathway like small molecule drugs.
Half life
Approximately 10-20 days (average 14 days).
Absorption
Subcutaneous bioavailability is approximately 64%. Peak serum concentrations are reached in 5 days.
Metabolism
Adalimumab is a protein, metabolized by catabolic pathways similar to endogenous IgG. Not metabolized by cytochrome P450 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to adalimumab or any excipient.
- •Active serious infections, including tuberculosis (TB), sepsis, and opportunistic infections.
- •Moderate to severe heart failure (NYHA Class III/IV).
Drug Interactions
Live vaccines
Avoid live vaccines during adalimumab treatment due to potential for infection.
Other biologic DMARDs (e.g., Anakinra, Abatacept)
Increased risk of serious infections when co-administered. Concomitant use is not recommended.
Storage
Store in a refrigerator at 2°C to 8°C (36°F to 46°F). Do not freeze. Protect from light. Keep out of the reach of children.
Overdose
No specific antidote for adalimumab overdose. Management should involve supportive care and treatment of symptoms.
Pregnancy & Lactation
Pregnancy: Use only if clearly needed after careful risk-benefit assessment. Lactation: Excreted into human milk at low levels. Consider developmental and health benefits of breastfeeding versus potential adverse effects on the breastfed child.
