Overview of the medicine
Adarbi 40 mg Tablet contains Azilsartan Medoxomil, an angiotensin II receptor blocker (ARB) used to treat high blood pressure (hypertension). It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Recommended starting dose is 40 mg once daily. Dosage may be increased to a maximum of 80 mg once daily if needed.
Elderly
No initial dose adjustment is necessary in patients over 65 years. However, caution should be exercised.
Renal_impairment
No initial dose adjustment is necessary for patients with mild to moderate renal impairment. For severe renal impairment, lower starting dose (e.g., 20 mg) may be considered with careful monitoring.
How to Take
Adarbi tablets should be taken orally once daily with or without food. Swallow the tablet whole with water.
Mechanism of Action
Azilsartan medoxomil is a prodrug that is rapidly converted to azilsartan, the active moiety, which is an orally active, selective angiotensin II subtype 1 (AT1) receptor antagonist. It blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II.
Pharmacokinetics
Onset
Blood pressure lowering effect is seen within 2 weeks, full effect in 2-4 weeks.
Excretion
Primarily excreted via feces (approximately 55%) and urine (approximately 42%), mostly as unchanged drug and metabolites.
Half life
Approximately 11 hours.
Absorption
Rapidly absorbed after oral administration; peak plasma concentration reached in 1.5-3 hours. Bioavailability is approximately 60%.
Metabolism
Metabolized to inactive metabolites mainly by O-dealkylation. No significant CYP450 metabolism.
Side Effects
Contraindications
- Hypersensitivity to azilsartan medoxomil or any component of the formulation
- Concomitant use with aliskiren in patients with diabetes or renal impairment
- Pregnancy (second and third trimesters)
Drug Interactions
NSAIDs
May reduce the antihypertensive effect of ARBs and increase the risk of renal function deterioration.
Lithium
Increased serum lithium concentrations and lithium toxicity have been reported with concomitant use of ARBs.
Aliskiren
Contraindicated in diabetic patients or those with renal impairment when used concomitantly.
Potassium-sparing diuretics/Potassium supplements
May lead to increased serum potassium levels.
Storage
Store at room temperature (below 30°C), protected from moisture and light. Keep out of reach of children.
Overdose
Limited data on overdose in humans. The most likely manifestation would be hypotension (low blood pressure). Treatment is supportive and symptomatic, including intravenous fluids if hypotension occurs. Azilsartan is not dialyzable.
Pregnancy & Lactation
Pregnancy Category D. Use of Adarbi is contraindicated during the second and third trimesters of pregnancy due to risks of fetal injury and death. Discontinue immediately if pregnancy is detected. It is not known whether azilsartan is excreted in human milk, caution is advised during breastfeeding.
