Description
Overview of the medicine
Afluzole 50 mg Suspension is an oral antifungal medication used to treat various fungal infections. It is particularly formulated as a suspension, making it suitable for pediatric patients and those who have difficulty swallowing tablets.
Uses & Indications
Dosage
Adults
For specific indications, 50-100 mg orally once daily for 7-14 days, or as directed by physician. Generally, higher strength formulations are used for adults.
Elderly
Similar to adult dosage, but monitor renal function carefully.
Renal_impairment
CrCl 21-50 mL/min: reduce dose by 50%. CrCl ≤20 mL/min: reduce dose by 75%.
How to Take
Administer orally. Shake well before use. Can be taken with or without food. Use an oral syringe or measuring spoon for accurate dosing, especially for children.
Mechanism of Action
Afluzole works by inhibiting the fungal cytochrome P450-dependent enzyme lanosterol 14-alpha-demethylase, which is essential for ergosterol biosynthesis. Ergosterol is a vital component of the fungal cell membrane, and its inhibition leads to increased cell membrane permeability, leakage of cellular contents, and ultimately cell death.
Pharmacokinetics
Onset
Clinical effects usually observed within 24-48 hours.
Excretion
Mainly excreted renally, with a small portion via feces.
Half life
Approximately 20-30 hours, allowing for once-daily dosing.
Absorption
Rapidly and well absorbed after oral administration; bioavailability is generally high and unaffected by food.
Metabolism
Primarily metabolized in the liver via CYP450 enzymes, though a significant portion is excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to afluzole or other azole antifungals
- Co-administration with terfenadine, astemizole, cisapride, pimozide, quinidine, or erythromycin due to risk of QTc prolongation
Drug Interactions
Warfarin
Increased prothrombin time, monitor INR.
Phenytoin
Increased phenytoin levels, monitor for toxicity.
Rifampicin
Decreased afluzole levels, may require dose adjustment.
Cyclosporine
Increased cyclosporine levels, monitor renal function.
Storage
Store below 30°C in a dry place, away from direct sunlight. Do not freeze. Keep out of reach of children.
Overdose
In case of overdose, supportive measures should be initiated. Gastric lavage may be considered. Hemodialysis removes afluzole from the body. Monitor vital signs and liver function.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Small amounts may pass into breast milk; caution advised during lactation. Consult a doctor.

Lab Monitoring
- Liver function tests (LFTs) periodically, especially during prolonged treatment
- Renal function tests if impairment is suspected or present
- Electrolyte levels (potassium, magnesium) if QTc prolongation risk factors are present
Doctor Notes
- Emphasize proper dosing and adherence, especially in pediatric patients.
- Advise patients about potential liver toxicity and need for monitoring.
- Review concomitant medications for potential drug interactions before prescribing.
Patient Guidelines
- Complete the full course of medication, even if symptoms improve.
