Overview of the medicine
AirPro (Formoterol Fumarate) is a long-acting beta-2 adrenergic agonist (LABA) used for the long-term maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD). It helps to relax the muscles around the airways, making it easier to breathe. It is not intended for acute relief of bronchospasm.
Uses & Indications
Dosage
Adults
For asthma and COPD: 12 mcg (2 inhalations of 6 mcg) twice daily. For EIB: 12 mcg (2 inhalations of 6 mcg) at least 15 minutes before exercise; do not exceed 2 doses per 24 hours.
Elderly
No dosage adjustment specifically required, but use with caution due to potential increased sensitivity.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
For oral inhalation only. Patients should be instructed on the proper use of the inhaler device. Do not swallow. Rinse mouth with water after each use to reduce the risk of oral candidiasis.
Mechanism of Action
Formoterol selectively stimulates beta-2 adrenergic receptors in the bronchial smooth muscle, leading to bronchodilation. It acts intracellularly to activate adenylyl cyclase, which catalyzes the conversion of ATP to cyclic-3',5'-adenosine monophosphate (cAMP). Increased cAMP levels cause relaxation of bronchial smooth muscle and inhibit the release of mediators from mast cells in the airways.

Onset
Within 1-3 minutes after inhalation.
Excretion
Mainly via urine (60%) and feces (32%) over 6-8 days.
Half life
Approximately 10-14 hours (terminal half-life).
Absorption
Rapidly absorbed after oral inhalation; peak plasma concentrations reached within 5-10 minutes. Absolute bioavailability is approximately 10%.
Metabolism
Primarily metabolized in the liver via direct glucuronidation and O-demethylation, followed by glucuronidation.
Side Effects
Contraindications
- Primary treatment of status asthmaticus or other acute episodes of asthma or COPD where intensive measures are required.
- Hypersensitivity to formoterol or any component of the formulation.
Drug Interactions
Beta-blockers
May blunt the pulmonary effect of AirPro. Avoid concomitant use unless benefits outweigh risks.
QTc-prolonging drugs
Increased risk of ventricular arrhythmias. Use with caution.
Diuretics (loop or thiazide)
May potentiate hypokalemia and ECG changes, especially in concomitant hypoxemia. Use with caution.
MAO Inhibitors, Tricyclic Antidepressants
May potentiate the effect of AirPro on the cardiovascular system. Use with extreme caution.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.

Symptoms of overdose include excessive beta-adrenergic stimulation (e.g., tachycardia, palpitations, tremor, headache, nausea, vomiting, hypokalemia, hyperglycemia). Treatment is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether formoterol is excreted in human milk; use with caution in nursing mothers.
