Description
Overview of the medicine
Alergix-MR is a combination medicine containing Desloratadine, an antihistamine, and Montelukast, a leukotriene receptor antagonist. It is used to relieve symptoms of allergic rhinitis and to prevent and treat chronic asthma.
Uses & Indications
Dosage
Adults
One tablet (Desloratadine 5 mg + Montelukast 10 mg) once daily, preferably in the evening.
Elderly
No specific dose adjustment is generally required, but caution is advised in patients with severe renal or hepatic impairment.
Renal_impairment
Use with caution. For severe renal impairment, dose adjustment or increased dosing interval may be considered for Desloratadine. Montelukast generally does not require adjustment.
How to Take
Take orally with or without food, preferably in the evening.
Mechanism of Action
Desloratadine is a potent, selective peripheral H1-receptor antagonist that provides non-sedating antihistamine activity. Montelukast is a selective and orally active leukotriene receptor antagonist that inhibits the cysteinyl leukotriene CysLT1 receptor, thereby reducing inflammatory effects.

Onset
Desloratadine: within 30 minutes; Montelukast: within 1 day for chronic effects.
Excretion
Desloratadine is excreted equally in urine and feces as metabolic products. Montelukast and its metabolites are primarily excreted in the feces via the bile.
Half life
Desloratadine: approximately 27 hours; Montelukast: 2.7 to 5.5 hours.
Absorption
Desloratadine is well absorbed, reaching peak plasma concentrations in approximately 3 hours. Montelukast is rapidly absorbed after oral administration, with peak plasma concentrations achieved in 2-4 hours for film-coated tablets.
Metabolism
Desloratadine is extensively metabolized to 3-hydroxydesloratadine. Montelukast is extensively metabolized in the liver by CYP2C8, 3A4, and 2C9.

Contraindications
- Hypersensitivity to Desloratadine, Montelukast, or any component of the tablet
- Severe liver disease (for Montelukast)
- Acute asthma attacks (Montelukast is not for acute relief)
Drug Interactions
Gemfibrozil
May increase Montelukast plasma concentrations.
Phenobarbital, Rifampicin
May decrease plasma concentration of Montelukast.
Ketoconazole, Erythromycin, Fluoxetine
May increase plasma concentration of Desloratadine.
