Uses & Indications
Dosage
Adults
Typically, the dosage of Diclofenac varies from 75 mg to 150 mg daily, divided into 2-3 doses. For Alflam 400 mg tablet, one tablet once daily or as advised by physician for specific conditions requiring higher doses, considering individual patient needs and tolerability.
Elderly
Lower doses may be required due to increased sensitivity to adverse effects and potential impaired renal function. Use with caution.
Renal_impairment
Use with caution in patients with mild to moderate renal impairment. Dose adjustment may be necessary. Contraindicated in severe renal impairment.
How to Take
Take the tablet orally, preferably with food or milk to minimize gastrointestinal upset. Swallow the tablet whole with a glass of water; do not crush, chew, or break it.
Mechanism of Action
Diclofenac works by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, which are involved in the synthesis of prostaglandins. Prostaglandins are mediators of inflammation, pain, and fever.

Contraindications

Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased methotrexate toxicity.
Corticosteroids
Increased risk of gastrointestinal ulceration and bleeding.
Diuretics and ACE inhibitors
Reduced hypotensive effects; risk of renal impairment increased.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. In severe cases, acute renal failure, respiratory depression, and coma may occur. Management involves gastric lavage, activated charcoal, and symptomatic and supportive treatment. There is no specific antidote.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to potential fetal cardiovascular effects. Use in the first and second trimesters and during lactation only if the potential benefit outweighs the potential risk. Consult a doctor.
