Overview of the medicine
Alfumax-ER 10 mg Tablet contains Alfuzosin, an alpha-1 blocker used to treat the symptoms of benign prostatic hyperplasia (BPH). It works by relaxing the muscles in the prostate and bladder neck, which helps to improve urine flow and reduce BPH symptoms.
Uses & Indications
Dosage
Adults
10 mg once daily, taken immediately after the same meal each day. The tablet should be swallowed whole.
Elderly
No dosage adjustment is typically required, but careful monitoring for hypotension is advised.
Renal_impairment
Mild to moderate renal impairment: No dosage adjustment. Severe renal impairment: Use with caution and monitor closely.
How to Take
Take orally, once daily, immediately after a meal. The tablet must be swallowed whole with liquid; it should not be crushed, chewed, or divided.
Mechanism of Action
Alfuzosin is a selective alpha-1 adrenergic receptor antagonist. It works by blocking alpha-1 receptors in the smooth muscle of the prostate, bladder neck, and prostatic capsule, leading to relaxation of these muscles. This relaxation decreases resistance to urine flow, thereby improving lower urinary tract symptoms (LUTS) associated with BPH.

Onset
Symptom improvement may be observed within 1-2 weeks of treatment.
Excretion
Mainly excreted in feces (75-91%) and to a lesser extent in urine (11%) as metabolites.
Half life
Approximately 5-8 hours.
Absorption
Well absorbed orally; absorption is increased by food. Peak plasma concentrations are reached about 6-8 hours after administration.
Metabolism
Extensively metabolized in the liver, primarily by the CYP3A4 enzyme.
Side Effects
Contraindications
- •Hypersensitivity to alfuzosin or any component of the tablet.
- •Severe hepatic impairment.
- •Concomitant use with other alpha-1 adrenergic blockers.
- •Concomitant use with potent CYP3A4 inhibitors (e.g., ketoconazole, itraconazole, ritonavir).
Drug Interactions
Antihypertensive drugs
Increased risk of hypotension, especially orthostatic hypotension.
Potent CYP3A4 inhibitors
May significantly increase alfuzosin plasma concentrations, leading to increased risk of adverse effects including hypotension.
PDE5 inhibitors (e.g., sildenafil, tadalafil)
Concomitant use may increase the risk of symptomatic hypotension.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to hypotension and reflex tachycardia. Management involves supportive measures, keeping the patient supine, and if necessary, vasopressors. Dialysis is unlikely to be beneficial due to high protein binding.
Pregnancy & Lactation
Alfuzosin is not indicated for use in women.
