Description
Overview of the medicine
Algaba 50 mg capsule contains Pregabalin, which is used to treat various types of neuropathic pain, fibromyalgia, and as an adjunctive therapy for partial-onset seizures and generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Initial dose is typically 75 mg twice daily or 50 mg three times daily, titrated up to a maximum of 300 mg twice daily or 600 mg daily based on response and tolerability. Dosage depends on the specific indication.
Elderly
Dosage adjustment may be necessary due to reduced renal function; start with lower doses and titrate carefully.
Renal_impairment
Dose must be reduced in patients with impaired renal function based on creatinine clearance (e.g., 25-50 mg daily for CrCl 30-60 mL/min).
How to Take
Take orally with or without food. Swallow the capsule whole with water. Do not crush, chew, or open the capsule.
Mechanism of Action
Pregabalin binds with high affinity to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system. This binding modulates the release of several excitatory neurotransmitters, including glutamate, norepinephrine, and substance P, leading to its anticonvulsant, anxiolytic, and analgesic effects.
Pharmacokinetics
Onset
Within 1 week for neuropathic pain; may take longer for other indications.
Excretion
Primarily excreted unchanged in the urine; renal elimination is the major route.
Half life
Approximately 6.3 hours
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations reached within 1.5 hours.
Metabolism
Negligible hepatic metabolism; less than 0.1% of the dose is metabolized in humans.
Side Effects
Contraindications
- Hypersensitivity to Pregabalin or any component of the formulation.
Drug Interactions
CNS Depressants (e.g., alcohol, benzodiazepines, opioids)
May potentiate the sedative and CNS depressant effects of Pregabalin, leading to increased drowsiness and impaired alertness.
Thiazolidinedione Antidiabetics (e.g., pioglitazone, rosiglitazone)
May increase the risk of weight gain and peripheral edema when co-administered with Pregabalin.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and restlessness. Treatment is supportive and symptomatic; hemodialysis can be used for significant overdose if necessary to remove Pregabalin from the system.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding as Pregabalin is excreted in human milk.

Lab Monitoring
- Renal function (creatinine clearance) in patients with kidney impairment. Periodic weight monitoring.
Doctor Notes
- Counsel patients on potential CNS depressant effects and the importance of not discontinuing suddenly due to withdrawal risks.
- Monitor renal function, especially in elderly patients and those with pre-existing kidney impairment; adjust dosage accordingly.
- Assess for signs of abuse, dependence, or suicidal ideation regularly, particularly in high-risk patients.
Patient Guidelines
- Do not stop taking Algaba suddenly, as this may lead to withdrawal symptoms such as insomnia, nausea, headache, and diarrhea. Consult your doctor for gradual dose reduction.
- Report any changes in mood or behavior, including suicidal thoughts, to your doctor immediately.
- Avoid driving or operating heavy machinery until you know how this medicine affects you, due to potential dizziness and somnolence.
Missed Dose Advice
If a dose is missed, take it as soon as you remember. If it is almost time for the next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Algaba may cause dizziness, somnolence, and blurred vision, which can impair your ability to drive or operate machinery. Patients should be advised not to drive or operate complex machinery until they are certain that Algaba does not affect their ability to perform such activities safely.
Lifestyle Advice
- Avoid alcohol consumption while on this medication, as it may enhance dizziness, drowsiness, and other central nervous system side effects.
- Maintain a healthy lifestyle, including regular exercise and a balanced diet, to support overall well-being and manage weight changes.
- Inform your doctor about all other medications you are taking, including over-the-counter drugs and herbal supplements.
