Description
Overview of the medicine
Algifast 500 mg suspension contains Paracetamol, an analgesic and antipyretic, used for pain relief and fever reduction in children and adults.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg (10 ml to 20 ml of 500 mg/5 ml suspension) every 4-6 hours as needed, not exceeding 4000 mg in 24 hours.
Elderly
No specific dose reduction required unless hepatic or renal impairment is present. Max 3000 mg/day recommended for long-term use.
Renal_impairment
Dose interval should be extended in patients with severe renal impairment (e.g., creatinine clearance <10 ml/min).
How to Take
Administer orally, preferably with or after food to minimize gastric irritation. Shake the bottle well before use. Use a measuring spoon or oral syringe for accurate dosing.
Mechanism of Action
Paracetamol primarily inhibits prostaglandin synthesis in the central nervous system, leading to its analgesic and antipyretic effects. It has minimal anti-inflammatory action.
Pharmacokinetics
Onset
Within 30-60 minutes for analgesia and antipyresis.
Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
Approximately 1 to 4 hours in adults; slightly shorter in children and slightly longer in neonates and patients with hepatic impairment.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30 to 60 minutes after oral administration.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation pathways. A small amount is metabolized by the cytochrome P450 enzyme system to N-acetyl-p-benzoquinone imine (NAPQI), a highly reactive intermediate.
Side Effects
Contraindications
- Hypersensitivity to Paracetamol or any component of the formulation
- Severe hepatic impairment
Drug Interactions
Alcohol
Increases risk of hepatotoxicity with chronic high-dose Paracetamol use.
Warfarin
May enhance the anticoagulant effect, increasing the risk of bleeding with prolonged high-dose Paracetamol use.
Cholestyramine
Reduces absorption of Paracetamol.
Metoclopramide, Domperidone
Increase absorption rate of Paracetamol.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children. Do not freeze.
Overdose
Overdose can lead to severe, potentially fatal hepatotoxicity. Symptoms include nausea, vomiting, abdominal pain, and delayed onset of liver damage. Treatment involves gastric lavage and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Paracetamol is generally considered safe for use during pregnancy and lactation at therapeutic doses, as it is widely used and well-tolerated. Consult a doctor.
