Overview of the medicine
Alistin-EF 600 mg Tablet contains Linezolid, an oxazolidinone antibiotic used to treat serious bacterial infections, particularly those caused by Gram-positive bacteria that are resistant to other antibiotics. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
600 mg orally every 12 hours for 10 to 14 consecutive days (or as directed by physician).
Elderly
No dose adjustment is generally required for elderly patients.
Renal_impairment
No dose adjustment is required for patients with renal impairment (including those on hemodialysis or peritoneal dialysis). However, accumulation of inactive metabolites may occur, and clinical monitoring is advised.
How to Take
Alistin-EF tablets can be taken with or without food. Swallow the tablet whole with water. Do not crush, chew, or break it.
Mechanism of Action
Linezolid inhibits bacterial protein synthesis by binding to the 23S ribosomal RNA of the 50S ribosomal subunit, thereby preventing the formation of a functional 70S initiation complex, which is essential for bacterial translation.
Pharmacokinetics
Onset
Peak plasma concentrations reached in 1-2 hours, indicating relatively rapid onset of action.
Excretion
Excreted primarily by non-renal pathways (65%) and renally (35%) as unchanged drug and metabolites.
Half life
Approximately 4-5 hours.
Absorption
Rapidly and extensively absorbed after oral administration, with absolute bioavailability of approximately 100%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Primarily metabolized by oxidation of the morpholine ring to two inactive carboxylic acid metabolites. It does not appear to induce or inhibit CYP450 enzymes.
Side Effects
Contraindications
- •Hypersensitivity to Linezolid or any component of the formulation.
- •Patients taking monoamine oxidase inhibitors (MAOIs) concurrently or within two weeks of MAOI therapy (due to potential for serotonin syndrome).
Drug Interactions
Rifampicin
May decrease linezolid concentrations, potentially reducing its efficacy.
Monoamine Oxidase Inhibitors (MAOIs)
Concurrent use is contraindicated due to risk of serotonin syndrome and hypertensive crisis.
Serotonergic agents (SSRIs, SNRIs, TCAs, Tramadol, Meperidine)
Increased risk of serotonin syndrome. Co-administration should be avoided or carefully monitored.
Adrenergic agents (e.g., pseudoephedrine, epinephrine, dopamine)
Linezolid may potentiate the vasopressor effects of these agents, leading to increased blood pressure.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, supportive care is recommended. Hemodialysis may remove approximately 30% of the linezolid dose in a 3-hour session, but there is no specific antidote.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Linezolid is excreted in human milk; a decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
