Description
Overview of the medicine
Albendazole is an anthelmintic medication used to treat a variety of parasitic worm infestations.
Uses & Indications
Dosage
Adults
For intestinal nematode infections (e.g., ascariasis, hookworm, pinworm, whipworm): 400 mg as a single dose. For strongyloidiasis or taeniasis: 400 mg once daily for 3 days. For neurocysticercosis or hydatid disease: 400 mg twice daily with meals for 8-30 days, depending on the condition.
Elderly
No specific dose adjustment is generally required, but close monitoring of renal and hepatic function is advised due to potential age-related organ impairment.
Renal_impairment
No dose adjustment is needed for patients with renal impairment as the drug and its metabolites are primarily excreted via bile. Use with caution in severe cases.
How to Take
The tablet can be chewed, crushed, or swallowed whole with water. It is recommended to take the medicine with a fatty meal to enhance absorption.
Mechanism of Action
Albendazole binds to beta-tubulin, inhibiting microtubule polymerization in parasitic helminths, leading to impaired glucose uptake and depletion of glycogen stores, ultimately immobilizing and killing the worms.
Pharmacokinetics
Onset
Variable, depending on the type and location of the worm infestation. Therapeutic levels of albendazole sulfoxide are generally achieved within 2-5 hours.
Excretion
Mainly excreted in the bile and, to a lesser extent, in the urine as metabolites.
Half life
The plasma half-life of albendazole is less than 1 hour; the active metabolite, albendazole sulfoxide, has a half-life of approximately 8-12 hours.
Absorption
Poorly absorbed from the gastrointestinal tract (less than 5%), but absorption is significantly increased (up to 5-fold) when taken with a fatty meal.
Metabolism
Extensively and rapidly metabolized in the liver to its primary active metabolite, albendazole sulfoxide, and then to albendazole sulfone.
Side Effects
Contraindications
- Hypersensitivity to albendazole or other benzimidazole derivatives.
- Pregnancy (especially first trimester) or suspicion of pregnancy.
- Patients with severe hepatic dysfunction.
Drug Interactions
Theophylline
Albendazole may decrease theophylline clearance, requiring monitoring of theophylline levels.
Cimetidine, Dexamethasone, Praziquantel
May increase plasma concentrations of albendazole sulfoxide, potentially leading to increased side effects.
Storage
Store in a cool, dry place below 30°C, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal disturbances, dizziness, headache, and fatigue. Treatment is symptomatic and supportive. Gastric lavage and activated charcoal may be considered within a few hours of ingestion.
Pregnancy & Lactation
Albendazole is contraindicated in the first trimester of pregnancy due to potential teratogenic effects. Use in later trimesters and during lactation only if the potential benefit justifies the potential risk to the fetus/infant and under strict medical supervision.

Lab Monitoring
- Liver function tests (LFTs) should be monitored periodically, especially during prolonged therapy.
- Complete blood count (CBC) should be monitored at the beginning of treatment and every two weeks during prolonged therapy (e.g., for neurocysticercosis or hydatid disease) due to the risk of bone marrow suppression.
Doctor Notes
- Emphasize the importance of taking with food, especially fatty meals, for optimal absorption.
- For prolonged courses, instruct patients on the necessity of regular liver function tests and complete blood counts.
Patient Guidelines
- Always complete the full course of treatment as prescribed by your doctor, even if symptoms improve.
- Take the tablet with food, preferably a fatty meal, to maximize absorption.
- Inform your doctor if you are pregnant, planning to become pregnant, or breastfeeding.
- Report any unusual or severe side effects to your doctor immediately.
Missed Dose Advice
If you miss a dose during a multi-day course, take it as soon as you remember, unless it is almost time for your next dose. Do not double the dose to catch up.
Driving Precautions
Albendazole may cause dizziness or lightheadedness in some individuals. If you experience these effects, avoid driving or operating machinery.
Lifestyle Advice
- Maintain good personal hygiene, especially hand washing, to prevent re-infection with intestinal worms.
- Ensure proper sanitation and avoid consuming contaminated food or water.
