Overview of the medicine
Aluric 100 mg Tablet contains Allopurinol, which is used to reduce the levels of uric acid in the body. It helps in the management of conditions caused by high uric acid, such as gout and certain kidney stones.
Uses & Indications
Dosage
Adults
Initial dose is usually 100 mg once daily. Dosage may be gradually increased by 100 mg at weekly intervals depending on serum uric acid levels, up to a maximum of 800 mg daily. Average maintenance dose is 200-300 mg daily.
Elderly
Lower initial doses are recommended. Start with 50-100 mg daily and titrate carefully, monitoring renal function.
Renal_impairment
Severe renal impairment (CrCl < 10 mL/min): Max 100 mg daily or 300 mg per week. Moderate renal impairment (CrCl 10-20 mL/min): Max 100-200 mg daily. Dose adjustments are crucial.
How to Take
Take orally, once daily, preferably after a meal to reduce gastric irritation. Swallow the tablet whole with a glass of water.
Mechanism of Action
Allopurinol acts as a xanthine oxidase inhibitor. It reduces the production of uric acid by inhibiting the enzyme xanthine oxidase, which is responsible for the conversion of hypoxanthine and xanthine to uric acid.
Pharmacokinetics
Onset
Reduction in serum uric acid levels generally begins within 24-48 hours, with maximal effects observed after several days to weeks.
Excretion
Approximately 70% of the daily ingested allopurinol (as allopurinol and oxypurinol) is excreted in the urine, mainly as oxypurinol.
Half life
Allopurinol has a plasma half-life of about 1-2 hours. Its active metabolite, oxypurinol, has a much longer half-life of about 15-20 hours.
Absorption
Approximately 80-90% of an oral dose is absorbed from the gastrointestinal tract.
Metabolism
Rapidly metabolized by xanthine oxidase to its major active metabolite, oxypurinol. Minor metabolism pathways also exist.
Side Effects
Contraindications
- •Hypersensitivity to allopurinol or any component of the formulation.
- •Should not be initiated during an acute attack of gout (wait until the acute attack has subsided).
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin; INR monitoring is recommended.
Cyclosporine
May increase cyclosporine plasma concentrations.
Thiazide Diuretics
Increased risk of allopurinol hypersensitivity, especially in patients with renal impairment.
Ampicillin/Amoxicillin
Increased risk of skin rash when taken concurrently with allopurinol.
Azathioprine, Mercaptopurine
Allopurinol inhibits metabolism of these drugs, necessitating a significant dose reduction (typically to 1/3 to 1/4 of the usual dose).
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, supportive treatment should be given. Maintain adequate hydration to promote uric acid excretion. Dialysis may be useful in severe cases to remove allopurinol and oxypurinol from the blood.
Pregnancy & Lactation
Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Allopurinol and oxypurinol are excreted in breast milk; use with caution in nursing mothers.
