Overview of the medicine
Amdocal Plus 5 mg Tablet is a combination medicine used to treat high blood pressure (hypertension) and high cholesterol. It contains Amlodipine, a calcium channel blocker, which relaxes blood vessels, and Atorvastatin, an HMG-CoA reductase inhibitor, which reduces the production of cholesterol in the body.
Uses & Indications
Dosage
Adults
One tablet once daily, preferably at the same time each day. Dosage may be adjusted based on individual response and tolerability.
Elderly
No specific dosage adjustment is generally required, but caution is advised due to potential for increased sensitivity.
Renal_impairment
No dosage adjustment necessary for mild to moderate renal impairment. Caution with severe impairment.
How to Take
Orally, with or without food. Take the tablet whole with water. It is recommended to take it at the same time each day.
Mechanism of Action
Amlodipine acts by blocking calcium influx into vascular smooth muscle and myocardial cells, causing vasodilation and reducing peripheral vascular resistance. Atorvastatin selectively inhibits HMG-CoA reductase, an enzyme responsible for converting 3-hydroxy-3-methylglutaryl coenzyme A to mevalonate, a precursor of sterols, including cholesterol. This inhibition leads to a reduction in cholesterol synthesis and an increase in LDL receptors on the hepatocyte surface, resulting in increased catabolism of LDL.

Onset
Amlodipine: 6-12 hours for antihypertensive effect. Atorvastatin: Lipid-lowering effects within 2 weeks, maximal by 4 weeks.
Excretion
Amlodipine: Primarily excreted in urine as inactive metabolites. Atorvastatin: Primarily excreted in bile as metabolites.
Half life
Amlodipine: 30-50 hours. Atorvastatin: Approximately 14 hours (20-30 hours for HMG-CoA reductase inhibitory activity due to active metabolites).
Absorption
Amlodipine: Well absorbed, bioavailability 64-90%, peak plasma levels 6-12 hours. Atorvastatin: Rapidly absorbed, bioavailability approximately 12% due to first-pass metabolism, peak plasma levels 1-2 hours.
Metabolism
Amlodipine: Extensively metabolized in the liver to inactive metabolites. Atorvastatin: Extensively metabolized by CYP3A4 to active and inactive metabolites.
Side Effects
Contraindications

Drug Interactions
CYP3A4 Inducers (e.g., Rifampicin)

Cyclosporine, Gemfibrozil, Niacin, Colchicine
Increased risk of myopathy/rhabdomyolysis when co-administered with Atorvastatin.
CYP3A4 Inhibitors (e.g., Ketoconazole, Diltiazem, Macrolides, Protease Inhibitors)
Can increase plasma concentrations of both Amlodipine and Atorvastatin, increasing risk of side effects like myopathy/rhabdomyolysis for Atorvastatin.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Overdose of Amlodipine can cause excessive peripheral vasodilation with marked hypotension and reflex tachycardia. Overdose of Atorvastatin has no specific treatment; general supportive measures should be implemented.
Pregnancy & Lactation
Contraindicated during pregnancy and lactation. Women of childbearing potential should use effective contraception. If pregnancy occurs, discontinue immediately.
