Description
Overview of the medicine
Amikast 10 mg Tablet contains Montelukast, a leukotriene receptor antagonist. It is used to prevent asthma attacks, ease breathing problems caused by exercise, and reduce symptoms of seasonal and perennial allergic rhinitis.
Uses & Indications
Dosage
Adults
10 mg once daily, taken in the evening.
Elderly
No dosage adjustment necessary.
Renal_impairment
No dosage adjustment necessary.
How to Take
Take orally once daily in the evening, with or without food. For asthma, take in the evening. For allergic rhinitis, the time of administration can be individualized. For both asthma and allergic rhinitis, take in the evening.
Mechanism of Action
Montelukast selectively antagonizes the cysteinyl leukotriene 1 (CysLT1) receptor. Leukotrienes are potent inflammatory mediators released from various cells, including mast cells and eosinophils. They bind to CysLT1 receptors in the airways and on other pro-inflammatory cells, causing bronchoconstriction, mucus secretion, vascular permeability, and eosinophil recruitment. By blocking the CysLT1 receptor, montelukast inhibits these effects, leading to bronchodilation and reduced inflammation.
Pharmacokinetics
Onset
Within 1 day for asthma, within 1-2 days for allergic rhinitis.
Excretion
Primarily excreted in feces via bile (approximately 86%), very little in urine.
Half life
2.7 to 5.5 hours
Absorption
Rapidly absorbed after oral administration. Peak plasma concentration achieved in 2-4 hours.
Metabolism
Extensively metabolized in the liver by CYP3A4, CYP2C9, and CYP2C8.
Side Effects
Contraindications
- Hypersensitivity to montelukast or any component of the tablet
Drug Interactions
Rifampicin
May decrease montelukast plasma concentrations. Dosage adjustment is usually not required.
Gemfibrozil
Coadministration with gemfibrozil may increase montelukast plasma concentrations, but no dose adjustment is recommended.
Phenobarbital
May decrease montelukast plasma concentrations.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose usually include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown harm, but there are no adequate and well-controlled studies in pregnant women. Use during pregnancy only if clearly needed. It is unknown if montelukast is excreted in human milk; caution should be exercised when administered to a nursing mother.
