Description
Overview of the medicine
Amlonaaf 5 mg Tablet contains Amlodipine, a calcium channel blocker used to treat high blood pressure (hypertension) and chest pain (angina). It works by relaxing blood vessels, allowing blood to flow more easily.
Uses & Indications
Dosage
Adults
Initial dose 5 mg once daily, maximum 10 mg once daily.
Elderly
Initial dose 2.5 mg once daily, maximum 10 mg once daily.
Renal_impairment
No dosage adjustment required.
How to Take
Take orally with or without food, usually once daily. Swallow the tablet whole with water.
Mechanism of Action
Amlodipine is a dihydropyridine calcium antagonist (calcium ion antagonist or slow channel blocker) that inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This causes peripheral arterial vasodilation, leading to reduced peripheral vascular resistance and blood pressure.
Pharmacokinetics
Onset
24-48 hours (full antihypertensive effect)
Excretion
Primarily excreted via urine (60% as metabolites, 10% unchanged drug), remaining via feces.
Half life
30-50 hours (terminal elimination half-life)
Absorption
Well absorbed after oral administration, bioavailability 64-90%. Peak plasma concentrations 6-12 hours.
Metabolism
Extensively metabolized in the liver to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to amlodipine or other dihydropyridine derivatives
- Severe hypotension
- Cardiogenic shock
- Unstable angina
- Severe aortic stenosis
Drug Interactions
Sildenafil
May enhance hypotensive effects.
Simvastatin
Limit simvastatin dose to 20 mg/day when co-administered with amlodipine.
CYP3A4 Inducers
May decrease amlodipine plasma concentrations.
CYP3A4 Inhibitors
May increase amlodipine plasma concentrations, requiring dose reduction.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management involves symptomatic treatment and cardiovascular support, including leg elevation, fluid administration, and vasopressors if necessary. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Not recommended during breastfeeding due to unknown excretion in human milk. Consult a doctor.
