Analpain 500 mg Tablet contains Paracetamol, which is a widely used medication for relieving mild to moderate pain and reducing fever. It works by inhibiting prostaglandin synthesis in the central nervous system.
Uses & Indications
Dosage
Adults
500 mg to 1000 mg (1-2 tablets) every 4-6 hours as needed. Maximum 4000 mg (8 tablets) in 24 hours.
Elderly
No specific dose reduction required, but caution is advised in frail elderly or those with impaired hepatic/renal function.
Renal_impairment
Dose adjustment may be necessary in severe renal impairment (creatinine clearance <10 mL/min). Interval prolongation to 6-8 hours.
How to Take
Take orally with or without food. Do not crush or chew the tablet. For best absorption, it can be taken on an empty stomach, but taking with food may reduce gastric upset.
Mechanism of Action
Paracetamol is thought to primarily act in the central nervous system by inhibiting cyclooxygenase (COX) enzymes, particularly COX-2, leading to reduced prostaglandin synthesis. This results in analgesic (pain-relieving) and antipyretic (fever-reducing) effects. Its peripheral anti-inflammatory activity is minimal.

Excretion
Mainly excreted in the urine as glucuronide and sulfate conjugates. Less than 5% is excreted unchanged.
Half life
1.5 to 3 hours (adults); slightly shorter in children, longer in neonates and in hepatic dysfunction.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 30 to 60 minutes after oral administration.
Metabolism
Primarily metabolized in the liver via glucuronidation and sulfation. A small portion is metabolized by cytochrome P450 to a highly reactive intermediate (N-acetyl-p-benzoquinone imine or NAPQI), which is detoxified by conjugation with glutathione.

Contraindications

Drug Interactions
Alcohol
Increased risk of hepatotoxicity with chronic excessive alcohol intake.
Warfarin and other coumarins
May enhance anticoagulant effect with prolonged regular daily use, increasing bleeding risk.
Phenobarbital, Phenytoin, Carbamazepine
May increase the risk of hepatotoxicity.

Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms include nausea, vomiting, abdominal pain, anorexia, and malaise. Severe overdose can lead to acute liver failure (hepatotoxicity), renal tubular necrosis, and coma. Treatment involves gastric decontamination (if recent) and administration of N-acetylcysteine (NAC) as an antidote.
Pregnancy & Lactation
Generally considered safe for use during all trimesters of pregnancy at recommended doses, but always consult a doctor. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding at recommended doses.
