Description
Overview of the medicine
Ancipro 500 mg Tablet contains Ciprofloxacin, a fluoroquinolone antibiotic used to treat various bacterial infections including urinary tract infections, respiratory tract infections, skin and soft tissue infections, bone and joint infections, and infectious diarrhea.
Uses & Indications
Dosage
Adults
The usual adult dose for most infections is 500 mg orally twice daily. Dosage may vary depending on the severity and type of infection, ranging from 250 mg to 750 mg twice daily. Duration of treatment depends on the infection.
Elderly
No specific dose adjustment is needed based solely on age, but renal function should be assessed, and dosage adjusted accordingly.
Renal_impairment
Dosage adjustment is necessary for patients with impaired renal function. For creatinine clearance between 30-60 mL/min, 250-500 mg every 12 hours. For <30 mL/min, 250-500 mg every 18-24 hours.
How to Take
Take Ancipro 500 mg Tablet orally, with or without food. However, to minimize potential gastrointestinal upset, it can be taken with food. Avoid taking it with dairy products (milk, yogurt) or calcium-fortified juices, as these can reduce absorption. Also, take it at least 2 hours before or 6 hours after antacids, iron, or zinc supplements.
Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.

Onset
Significant plasma levels observed within 1-2 hours of oral administration.
Excretion
Primarily renal, with approximately 50-70% of an oral dose excreted unchanged in the urine. A smaller portion is eliminated via the feces.
Half life
Approximately 3-5 hours.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, with bioavailability of approximately 70-80%. Peak plasma concentrations are reached within 1-2 hours.
Metabolism
Undergoes limited hepatic metabolism (approximately 15-30%) via CYP450 enzymes to form less active metabolites.
Side Effects
Contraindications
- Hypersensitivity to ciprofloxacin or other fluoroquinolones.
- Concomitant administration with tizanidine.
- Children and adolescents (except in specific conditions like complicated UTIs, anthrax, or cystic fibrosis).
Drug Interactions
NSAIDs
May increase the risk of CNS stimulation and convulsive seizures.
Warfarin
May enhance the anticoagulant effect of warfarin, requiring INR monitoring.
Tizanidine
Concomitant use is contraindicated due to increased tizanidine levels, leading to hypotension and sedation.
Theophylline
Ciprofloxacin can increase theophylline levels, leading to toxicity. Monitor theophylline levels.
Antacids/Iron/Zinc
Can reduce ciprofloxacin absorption. Administer at least 2 hours before or 6 hours after.
