Description
Overview of the medicine
Ancor Plus 5 mg Tablet is a combination medication containing Amlodipine and Atenolol. It is primarily used to treat high blood pressure (hypertension) and angina (chest pain). Amlodipine is a calcium channel blocker that relaxes blood vessels, and Atenolol is a beta-blocker that reduces heart rate and the heart's pumping force, thereby lowering blood pressure and easing the heart's workload.
Uses & Indications
Dosage
Adults
Typically one Ancor Plus 5 mg tablet orally once daily. Dosage may be adjusted based on individual response and tolerability, as directed by a physician.
Elderly
Elderly patients may require lower initial doses and careful titration due to decreased renal and hepatic function. Consult a physician for dosage adjustment.
Renal_impairment
No specific dosage adjustment is typically required for mild to moderate renal impairment. For severe impairment, caution is advised and dose adjustment may be necessary, particularly for Atenolol. Consult a physician.
How to Take
Take the tablet orally, with or without food, usually once daily at the same time each day. Swallow the tablet whole with water; do not crush, chew, or break it.
Mechanism of Action
Amlodipine blocks calcium ion influx across cardiac and vascular smooth muscle, leading to peripheral vasodilation and reduced peripheral vascular resistance. Atenolol selectively blocks beta-1 adrenergic receptors, resulting in decreased heart rate, myocardial contractility, and blood pressure. Together, they synergistically lower blood pressure and reduce myocardial oxygen demand.
Pharmacokinetics
Onset
Amlodipine: Gradual onset (6-12 hours). Atenolol: Rapid onset (within 1 hour).
Excretion
Amlodipine: Primarily renal excretion of inactive metabolites (60%). Atenolol: Primarily renal excretion of unchanged drug (50%).
Half life
Amlodipine: 30-50 hours. Atenolol: 6-9 hours.
Absorption
Amlodipine: Well absorbed, high oral bioavailability (64-90%). Atenolol: Incompletely absorbed (50-60% oral bioavailability).
Metabolism
Amlodipine: Extensively metabolized in the liver to inactive metabolites. Atenolol: Minimally metabolized in the liver (less than 10%).
Side Effects
Contraindications
- •Hypersensitivity to Amlodipine, Atenolol, or any component of the formulation
- •Cardiogenic shock
- •Decompensated heart failure
- •Second- or third-degree atrioventricular (AV) block without a pacemaker
- •Severe bradycardia
- •Sick sinus syndrome (unless a pacemaker is in place)
- •Severe hypotension
- •Severe peripheral arterial disease
- •Metabolic acidosis
Drug Interactions
Digitalis Glycosides
Increased risk of bradycardia and AV block.
Other Antihypertensives
May cause additive hypotensive effects, potentially leading to orthostatic hypotension.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May reduce the antihypertensive effect of Atenolol.
CYP3A4 Inducers (e.g., Rifampicin, Carbamazepine)
May decrease Amlodipine exposure, reducing its efficacy.
CYP3A4 Inhibitors (e.g., Ketoconazole, Erythromycin)
May increase Amlodipine exposure, leading to increased risk of hypotension.
Class I Antiarrhythmics (e.g., Disopyramide, Quinidine)
Increased risk of myocardial depression and bradycardia.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe hypotension, bradycardia, heart failure, and bronchospasm. Management involves symptomatic and supportive treatment, including gastric lavage, activated charcoal, and IV fluids for hypotension. Glucagon or atropine may be used for bradycardia, and a beta2-agonist for bronchospasm.
Pregnancy & Lactation
Ancor Plus is generally not recommended during pregnancy or lactation unless the potential benefits outweigh the potential risks to the fetus or infant. Atenolol is pregnancy category D, and Amlodipine is category C. Consult a physician before use.
