Description
Overview of the medicine
Angical 5 mg Tablet contains Amlodipine, a long-acting calcium channel blocker used to treat high blood pressure (hypertension) and chest pain (angina). It works by relaxing blood vessels, making it easier for the heart to pump blood and reducing the heart's workload.
Uses & Indications
Dosage
Adults
Initial dose is usually 5 mg once daily. Maximum dose is 10 mg once daily. Dose may be adjusted based on patient response.
Elderly
A lower initial dose of 2.5 mg once daily may be considered; adjust cautiously.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Take Angical 5 mg Tablet orally once daily, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Amlodipine inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle. This causes peripheral arterial vasodilation, which in turn lowers blood pressure and reduces myocardial workload, thus relieving angina.
Pharmacokinetics
Onset
6-12 hours for antihypertensive effect.
Excretion
Excreted via urine (approximately 60%) and feces (approximately 25%).
Half life
30-50 hours.
Absorption
Well absorbed orally, bioavailability 64-90%. Peak plasma concentrations within 6-12 hours.
Metabolism
Extensively hepatic metabolism to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to amlodipine, other dihydropyridines, or any component of the formulation.
- Cardiogenic shock.
- Severe aortic stenosis.
- Unstable angina or acute myocardial infarction.
Drug Interactions
Sildenafil
May potentiate the hypotensive effects of sildenafil.
Simvastatin
Limit simvastatin dose to 20 mg/day due to increased risk of myopathy/rhabdomyolysis.
CYP3A4 Inducers (e.g., rifampicin)
May decrease amlodipine exposure, reducing its effectiveness.
CYP3A4 Inhibitors (e.g., ketoconazole, erythromycin)
May increase amlodipine exposure, requiring dose reduction.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive peripheral vasodilation with marked hypotension and possibly reflex tachycardia. Management is symptomatic and supportive, including cardiovascular monitoring, IV fluids, and vasopressors if severe hypotension persists.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Small amounts of amlodipine are excreted in breast milk; consult your doctor before breastfeeding.
