Description
Overview of the medicine
Angitan 50 mg Tablet contains Losartan Potassium, an angiotensin II receptor blocker (ARB), used to treat high blood pressure (hypertension) and to protect the kidneys in patients with type 2 diabetes and hypertension.
Uses & Indications
Dosage
Adults
Hypertension: Initial 50 mg once daily, adjusted to 25-100 mg once daily. Diabetic nephropathy: Initial 50 mg once daily, adjusted to 100 mg once daily based on blood pressure response.
Elderly
No initial dosage adjustment typically required unless volume depleted or hepatic impaired.
Renal_impairment
No initial dosage adjustment is generally required. For patients with severe renal impairment (creatinine clearance <20 mL/min), a lower starting dose (e.g., 25 mg) may be considered.
How to Take
Angitan tablets can be taken with or without food, preferably at the same time each day.
Mechanism of Action
Losartan selectively blocks the binding of angiotensin II to the AT1 receptor found in many tissues (e.g., vascular smooth muscle, adrenal gland). This blockade prevents angiotensin II from causing vasoconstriction and aldosterone secretion, leading to a reduction in blood pressure.
Pharmacokinetics
Onset
Antihypertensive effect onset within 6 hours; full effect within 3-6 weeks.
Excretion
Approximately 35% excreted in urine and 60% in feces (biliary excretion) as unchanged drug and metabolites.
Half life
Losartan: 2 hours; active metabolite (E-3174): 6-9 hours.
Absorption
Well absorbed orally, approximately 33% bioavailability due to first-pass metabolism. Peak plasma concentrations of losartan are reached in 1 hour.
Metabolism
Extensively metabolized by cytochrome P450 enzymes (primarily CYP2C9 and CYP3A4) to an active carboxylic acid metabolite (E-3174) and several inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Losartan or any component of the formulation
- Pregnancy (especially second and third trimesters)
- Concomitant use with aliskiren in patients with diabetes or renal impairment
Drug Interactions
Lithium
Increased serum lithium concentrations and toxicity.
Aliskiren
Contraindicated in patients with diabetes or renal impairment (eGFR <60 mL/min/1.73m2). Increases risk of hypotension, hyperkalemia, and renal impairment.
NSAIDs (e.g., ibuprofen, naproxen)
May reduce the antihypertensive effect of losartan and increase the risk of renal function deterioration, especially in elderly or volume-depleted patients.
Diuretics (potassium-sparing) / Potassium supplements
May lead to increased serum potassium levels (hyperkalemia).
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are likely to be hypotension and tachycardia; bradycardia could occur from parasympathetic stimulation. Treatment is symptomatic and supportive. If ingestion is recent, induce emesis or perform gastric lavage. Activated charcoal may be used.
Pregnancy & Lactation
Pregnancy: Angitan is contraindicated during pregnancy due to fetal toxicity. If pregnancy is detected, discontinue immediately. Lactation: It is not known whether losartan is excreted in human milk. A decision should be made whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.

Lab Monitoring
- Serum potassium levels (especially in patients with renal impairment or on potassium-sparing diuretics)
- Renal function (BUN, creatinine, eGFR)
- Liver function tests (LFTs) periodically
Doctor Notes
- Emphasize the importance of consistent dosing and not abrupt discontinuation.
- Monitor blood pressure, renal function, and serum potassium regularly.
- Counsel female patients of childbearing potential about the fetal risk and use of contraception.
- Consider lower initial doses in patients with hepatic impairment or severe volume depletion.
Patient Guidelines
- Take Angitan as prescribed by your doctor, usually once daily.
- Do not stop taking this medicine without consulting your doctor, even if you feel well.
- Regularly monitor your blood pressure at home if advised by your doctor.
- Inform your doctor about all other medications, supplements, and herbal products you are taking.
