Description
Overview of the medicine
Anidulafungin is an echinocandin antifungal indicated for the treatment of invasive candidiasis, including candidemia, and esophageal candidiasis.
Uses & Indications
Dosage
Adults
Invasive Candidiasis: 200 mg loading dose on Day 1, then 100 mg once daily. Esophageal Candidiasis: 100 mg loading dose on Day 1, then 50 mg once daily.
Elderly
No dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is required for patients with renal impairment.
How to Take
Anidulafungin must be reconstituted with sterile water for injection and then diluted in 0.9% sodium chloride or 5% dextrose for intravenous infusion. Administer slowly over approximately 3 hours.
Mechanism of Action
Anidulafungin selectively inhibits 1,3-beta-D-glucan synthase, an enzyme essential for the synthesis of fungal cell wall glucan. This results in osmotic instability and lysis of fungal cells.
Pharmacokinetics
Onset
Rapid onset of action following IV infusion.
Excretion
Primarily eliminated via fecal excretion (non-renal clearance), with very little unchanged drug excreted in urine.
Half life
Approximately 24-26 hours.
Absorption
Administered intravenously, with 100% bioavailability.
Metabolism
Not metabolized by cytochrome P450 enzymes; undergoes slow chemical degradation.
Side Effects
Contraindications
- Hypersensitivity to anidulafungin or any other echinocandin.
- History of severe allergic reactions to components of the formulation.
Drug Interactions
Tacrolimus
No dosage adjustment needed. No significant interaction.
Cyclosporine
No dosage adjustment needed. Anidulafungin does not inhibit CYP450 enzymes.
Voriconazole
No dosage adjustment needed. No significant interaction.
Storage
Store unreconstituted vials refrigerated at 2°C to 8°C (36°F to 46°F). Protect from light. Do not freeze. Reconstituted solution should be diluted immediately and used within 24 hours.
Overdose
No specific antidote for anidulafungin overdose. Management should be supportive and symptomatic. Hemodialysis is not effective in removing anidulafungin.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. It is unknown if anidulafungin is excreted in human breast milk; use with caution during lactation.
