Description
Overview of the medicine
Anril-SR 26 mg Tablet contains Trimetazidine Dihydrochloride in a sustained-release formulation. It is used for the symptomatic treatment of stable angina pectoris, offering cardioprotective benefits by optimizing myocardial metabolism under ischemic conditions.
Uses & Indications
Dosage
Adults
One 26 mg modified-release tablet twice daily, during meals.
Elderly
No specific dose adjustment is required in elderly patients, but caution is advised due to potential age-related decrease in renal function. Monitor renal function.
Renal_impairment
In patients with moderate renal impairment (creatinine clearance 30-60 mL/min), the recommended dose is one 26 mg modified-release tablet in the morning only. Not recommended in severe renal impairment (CrCl < 30 mL/min).
How to Take
Oral administration. Tablets should be swallowed whole with a glass of water, during meals. Do not chew or crush the tablet.
Mechanism of Action
Trimetazidine acts by inhibiting beta-oxidation of fatty acids in ischemic cells, thereby shifting myocardial energy metabolism towards glucose oxidation. This preserves cellular ATP levels, reduces acidosis, and improves myocardial function during ischemia without affecting hemodynamic parameters.
Pharmacokinetics
Onset
Within 2 hours, peak plasma concentration around 5 hours for SR.
Excretion
Mainly renal, primarily unchanged in urine.
Half life
Approximately 7-12 hours for SR formulations.
Absorption
Well absorbed after oral administration. Sustained release profile provides consistent plasma concentrations over 24 hours.
Metabolism
Minimal hepatic metabolism, primarily excreted unchanged.
Side Effects
Contraindications
- Hypersensitivity to the active substance or any excipient
- Parkinson's disease, Parkinsonian symptoms, tremors, restless leg syndrome and other related movement disorders
- Severe renal impairment (creatinine clearance < 30 mL/min)
Drug Interactions
Levodopa
Trimetazidine may exacerbate Parkinsonian symptoms when co-administered with Levodopa. Concomitant use should be avoided.
Storage
Store in a cool and dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Limited experience with overdose. Treatment should be symptomatic and supportive. Gastric lavage may be considered.
Pregnancy & Lactation
Pregnancy: Limited data. Avoid during pregnancy unless absolutely necessary and benefits outweigh risks. Lactation: Unknown if excreted in breast milk. Breastfeeding should be discontinued during treatment.

Clinical Trials
Extensive clinical trials have demonstrated Trimetazidine's efficacy in improving exercise tolerance, reducing angina attacks, and improving left ventricular function in patients with stable angina. Studies like the TRIMPOL-2 and VASCO trials have supported its use as an add-on therapy.
Lab Monitoring
- Renal function tests (creatinine clearance) in elderly or renally impaired patients, especially before starting treatment.
Doctor Notes
- Consider Trimetazidine as an add-on therapy for patients with stable angina who are not adequately controlled with first-line anti-anginal agents or who have contraindications/intolerance to them.
- Educate patients about potential neurological side effects, especially Parkinsonian symptoms, and instruct them to report any new or worsening movement disorders immediately.
- Assess renal function before initiating treatment and adjust dosage in moderate renal impairment.
Patient Guidelines
- Take this medicine exactly as prescribed by your doctor.
- Do not stop taking it without consulting your doctor, even if you feel better.
- Inform your doctor about all other medicines you are taking, including herbal supplements.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
