Description
Overview of the medicine
Antanil 200 mg Suspension is an anthelmintic medication containing Mebendazole, used to treat various parasitic worm infections such as pinworm, roundworm, whipworm, and hookworm. It works by preventing the worms from absorbing sugar (glucose), which leads to the depletion of their energy stores and eventually their death.
Uses & Indications
Dosage
Adults
Pinworm: 200 mg single dose. Repeat in 2-4 weeks if necessary. Other infections (roundworm, whipworm, hookworm, mixed): 200 mg twice daily for 3 consecutive days.
Elderly
Same as adult dosage. No specific dose adjustment generally required, but caution with severe hepatic impairment.
Renal_impairment
No dosage adjustment needed for renal impairment as systemic absorption is minimal and excretion is primarily fecal.
How to Take
Shake well before use. The suspension can be taken with or without food. For better absorption, it can be taken with a fatty meal. No special diet or fasting is necessary.
Mechanism of Action
Mebendazole selectively and irreversibly blocks glucose uptake and other nutrients in susceptible helminths, leading to the depletion of glycogen stores. This results in decreased ATP formation, immobilization, and ultimately the death of the parasite. It primarily acts on the intestinal lumen.
Pharmacokinetics
Onset
Effects on worms begin within hours, but complete parasite clearance may take several days to weeks depending on the infection.
Excretion
The majority (approximately 90%) of the administered dose is excreted unchanged in the feces. A small portion of absorbed drug and its metabolites are excreted in the urine.
Half life
Plasma half-life is typically 2.5 to 5.5 hours. For its active metabolite (hydrolyzed mebendazole), it can be up to 10-15 hours.
Absorption
Poorly absorbed from the gastrointestinal tract (approximately 5-10%). Absorption can be increased when taken with a fatty meal.
Metabolism
Extensively metabolized in the liver, primarily by hydrolysis. The main metabolite is 2-amino-5(6)-benzimidazolyl carbamate.

Drug Interactions
Cimetidine
May increase plasma concentration of Mebendazole by inhibiting its metabolism, leading to increased systemic exposure.
Metronidazole
Concurrent use with Mebendazole has been reported to be associated with an increased risk of Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN).
Phenytoin, Carbamazepine
May decrease plasma concentration of Mebendazole due to induction of hepatic metabolism, potentially reducing its efficacy.

Overdose
In case of overdose, gastrointestinal symptoms (abdominal pain, nausea, vomiting, diarrhea) may occur. Treatment is symptomatic and supportive, including gastric lavage and activated charcoal if overdose is recent.

Pregnancy Category C. Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. It is recommended to avoid use during the first trimester. Mebendazole is minimally excreted in human milk; use with caution during lactation.
