Description
Overview of the medicine
Apetar 160 mg tablet contains Megestrol Acetate, a synthetic progestin primarily used to stimulate appetite and promote weight gain in patients suffering from anorexia, cachexia, or unexplained significant weight loss, especially in AIDS patients. It is also used in the palliative treatment of advanced breast and endometrial cancer.
Uses & Indications
Dosage
Adults
For anorexia/cachexia: 400-800 mg/day, typically administered as 160 mg (one tablet) four times daily, or as directed by physician. For advanced breast cancer: 160 mg once daily. For advanced endometrial carcinoma: 40-320 mg/day, typically 40-80 mg four times daily.
Elderly
No specific dose adjustment is generally required, but caution should be exercised due to potential age-related decrease in renal or hepatic function.
Renal_impairment
Use with caution. Specific dose adjustments for severe renal impairment have not been established, but elimination may be reduced.
How to Take
Apetar tablets should be taken orally. It can be taken with or without food, but absorption may be enhanced when taken with food. Follow the physician's instructions regarding dose and frequency.
Mechanism of Action
The exact mechanism of appetite stimulation is not fully understood but is thought to involve inhibition of cytokine production (e.g., TNF-alpha, IL-1, IL-6) and other inflammatory mediators that can contribute to cachexia. It also has glucocorticoid-like effects.
Pharmacokinetics
Onset
Appetite effects typically begin within 3-5 weeks of continuous therapy.
Excretion
Mainly renal (57-78%) as metabolites and unchanged drug, some fecal excretion (8-30%).
Half life
Highly variable, ranging from approximately 13 to 105 hours.
Absorption
Variable oral absorption, increased with food. Peak plasma concentrations are reached within 2-5 hours.
Metabolism
Primarily hepatic, undergoes hydroxylation and conjugation. Extensively metabolized.
Side Effects
Contraindications
- •Hypersensitivity to Megestrol Acetate or any component of the formulation.
- •Known or suspected pregnancy.
Drug Interactions
Warfarin
May decrease the anticoagulant effect of warfarin; INR monitoring is recommended.
Insulin/Oral Hypoglycemics
May increase blood glucose levels, requiring adjustment of antidiabetic medication.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive treatment should be initiated. There is no specific antidote. Hemodialysis is unlikely to be effective due to high protein binding.
Pregnancy & Lactation
Apetar is contraindicated during pregnancy due to potential fetal harm. It is not recommended during lactation as it is unknown if Megestrol Acetate is excreted in human milk, but potential adverse effects on the infant cannot be ruled out.
