Description
Overview of the medicine
Apidra SoloStar contains insulin glulisine, a rapid-acting insulin analogue used to improve glycemic control in adults, adolescents, and children with diabetes mellitus. It is administered by subcutaneous injection shortly before or soon after meals.
Uses & Indications
Dosage
Adults
Individualized dosage based on patient's metabolic needs, blood glucose monitoring, and glycemic control goals. Typically administered 0-15 minutes before or shortly after a meal.
Elderly
Dosage should be individualized. Careful monitoring of glucose may be necessary due to potential for greater insulin sensitivity or renal impairment.
Renal_impairment
Insulin requirements may be reduced in patients with renal impairment. Careful monitoring of glucose and dosage adjustment are necessary.
How to Take
Subcutaneous injection into the abdominal wall, thigh, or deltoid. Rotate injection sites within the same region to reduce the risk of lipodystrophy. Do not administer intravenously or intramuscularly.
Mechanism of Action
Insulin glulisine lowers blood glucose by stimulating peripheral glucose uptake, especially by skeletal muscle and fat, and by inhibiting hepatic glucose production. Insulin also inhibits lipolysis and proteolysis, and enhances protein synthesis.
Pharmacokinetics
Onset
Onset of action within 10-20 minutes.
Excretion
Excreted primarily by the kidneys.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed following subcutaneous injection due to amino acid substitutions (Lysine and Glutamic acid) leading to faster dissociation from hexamers into monomers.
Metabolism
Primarily metabolized by insulin degrading enzymes, mainly in the liver and kidney.
Side Effects
Contraindications
- Hypersensitivity to insulin glulisine or any of its excipients.
- Hypoglycemia (low blood sugar).
Drug Interactions
Beta-blockers, clonidine, lithium salts, alcohol
May mask symptoms of hypoglycemia.
Oral contraceptives, corticosteroids, thyroid hormones, diuretics, sympathomimetics
May reduce insulin's glucose-lowering effect.
ACE inhibitors, angiotensin II receptor blockers, disopyramide, fibrates, fluoxetine, MAO inhibitors, pentoxifylline, propoxyphene, salicylates, somatostatin analogues, sulfonamide antibiotics
May enhance insulin's glucose-lowering effect.
Storage
Unopened: Store in a refrigerator (2°C to 8°C). Do not freeze. Protect from light. Opened/In-use: Store below 25°C. Do not refrigerate or freeze. Discard after 28 days.
Overdose
Overdose of insulin glulisine leads to severe hypoglycemia. Management includes oral glucose for mild cases, or intravenous glucose/glucagon for severe cases, along with continuous glucose monitoring.
Pregnancy & Lactation
Use during pregnancy requires careful monitoring and dosage adjustment as insulin needs may change. Insulin does not pass into breast milk, so Apidra can be used during lactation with appropriate monitoring.

Lab Monitoring
- Blood glucose levels (fasting, pre-meal, post-meal, bedtime)
- HbA1c
- Kidney function (e.g., creatinine, GFR)
- Liver function tests (if clinically indicated)
Doctor Notes
- Emphasize patient education on injection technique and site rotation.
- Stress the importance of carbohydrate counting for optimal dosing.
- Regularly assess for signs of hypoglycemia and educate patients on its management.
Patient Guidelines
- Always check the insulin label before each injection.
- Rotate injection sites to prevent lipodystrophy.
- Do not reuse needles.
- Never share pens with other people.
- Store properly as instructed.
Missed Dose Advice
If a dose is missed, blood glucose should be checked, and the next dose should be taken at the next scheduled meal, if appropriate. Do not take a double dose to make up for a missed dose.
Driving Precautions
Patients should be advised to take precautions to avoid hypoglycemia while driving or operating machinery. Regular meals and blood glucose monitoring are essential.
Lifestyle Advice
- Maintain a consistent diet and exercise routine.
- Monitor blood glucose regularly.
- Carry a source of fast-acting glucose (e.g., glucose tablets, fruit juice) for hypoglycemia.
- Inform your doctor about all other medications you are taking.
