Description
Overview of the medicine
Apnac 50 mg Tablet contains Aceclofenac, a non-steroidal anti-inflammatory drug (NSAID) used to relieve pain and inflammation in conditions like osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis.
Uses & Indications
Dosage
Adults
The recommended dose is 100 mg twice daily (one 50 mg tablet twice daily or one 100 mg tablet twice daily). Dosage may be adjusted based on clinical response and tolerability.
Elderly
The elderly should be monitored closely and treated with the lowest effective dose for the shortest duration. No specific dose reduction is usually necessary unless there is renal or hepatic impairment.
Renal_impairment
In patients with mild to moderate renal impairment, caution is advised. A lower starting dose may be considered, and careful monitoring of renal function is recommended. Avoid in severe renal impairment.
How to Take
Apnac 50 mg Tablet should be swallowed whole with sufficient liquid, preferably with or after food, to minimize gastrointestinal discomfort.
Mechanism of Action
Aceclofenac exerts its anti-inflammatory and analgesic effects by inhibiting the enzyme cyclooxygenase (COX), which is involved in the synthesis of prostaglandins. Prostaglandins are mediators of inflammation, pain, and fever.
Pharmacokinetics
Onset
Onset of action for pain relief is typically within 1 hour.
Excretion
Approximately two-thirds of the administered dose is excreted in the urine, mainly as conjugated metabolites, and one-third in the faeces.
Half life
The plasma elimination half-life is approximately 4-4.3 hours.
Absorption
Well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached approximately 1.25 to 3 hours after ingestion.
Metabolism
Extensively metabolized in the liver, primarily via hydroxylation by CYP2C9, to its main metabolite 4'-hydroxyaceclofenac.
Side Effects
Contraindications
- Hypersensitivity to Aceclofenac or any other NSAID (e.g., aspirin)
- Patients with active gastrointestinal bleeding or peptic ulceration
- Severe renal or hepatic impairment
- Severe heart failure
- Third trimester of pregnancy
- History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs
Drug Interactions
Lithium
Increased plasma lithium levels.
Methotrexate
Increased plasma methotrexate levels and toxicity.
Diuretics and ACE inhibitors
Reduced hypotensive effect and increased risk of renal impairment.
Other NSAIDs and corticosteroids
Increased risk of gastrointestinal side effects.
Warfarin and other anticoagulants
Increased risk of bleeding.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include headache, nausea, vomiting, epigastric pain, gastrointestinal bleeding, diarrhea, disorientation, excitation, coma, drowsiness, dizziness, tinnitus, and convulsions. In cases of significant poisoning, acute renal failure and liver damage are possible. Treatment is symptomatic and supportive, including gastric emptying (e.g., activated charcoal or gastric lavage) within one hour of ingestion. Monitor vital signs and renal function.
Pregnancy & Lactation
Aceclofenac is contraindicated in the third trimester of pregnancy due to potential risks to the fetus (e.g., premature closure of the ductus arteriosus). In the first and second trimesters, it should be used only if the potential benefit outweighs the potential risk. Aceclofenac is excreted in human milk in small amounts; therefore, it is generally not recommended during breastfeeding.

Clinical Trials
Aceclofenac has undergone extensive clinical trials demonstrating its efficacy and safety in various inflammatory and painful conditions, especially in osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis. Studies often compare its effectiveness and tolerability with other NSAIDs.
Lab Monitoring
- Liver function tests (ALT, AST) especially with long-term use
- Renal function tests (Creatinine, BUN) with long-term use or in patients with risk factors
- Complete blood count (CBC) to monitor for blood dyscrasias
- Fecal occult blood test (if GI symptoms present)
Doctor Notes
- Consider lowest effective dose for shortest duration, especially in elderly or patients with cardiovascular/GI risk factors.
- Monitor for signs of GI bleeding (e.g., black tarry stools, coffee-ground vomitus) and cardiovascular events.
- Assess renal and hepatic function periodically during long-term therapy.
- Advise patients on potential dizziness and its implications for driving/operating machinery.
Patient Guidelines
- Take the tablet with food or milk to reduce stomach upset.
- Do not exceed the prescribed dose or duration of treatment.
- Report any unusual symptoms, especially stomach pain, black stools, or yellowing of the skin/eyes, to your doctor immediately.
- Avoid concomitant use with other NSAIDs or aspirin without consulting your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
Apnac may cause dizziness or visual disturbances in some patients. If you experience these effects, you should avoid driving or operating machinery.
Lifestyle Advice
- Avoid or limit alcohol consumption as it may increase the risk of stomach irritation and bleeding.
- Maintain adequate hydration, especially if you have kidney problems.
- If you experience dizziness, avoid driving or operating heavy machinery.
