Description
Overview of the medicine
Pantoprazole is a proton pump inhibitor used to treat conditions where stomach acid needs to be reduced, such as GERD, peptic ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
Erosive Esophagitis: 40 mg once daily for up to 8 weeks. Zollinger-Ellison: 40-80 mg twice daily.
Elderly
No specific dose adjustment generally needed, but monitor for adverse effects.
Renal_impairment
No dose adjustment required.
How to Take
Take orally, preferably 30-60 minutes before a meal. Swallow whole, do not crush or chew.
Mechanism of Action
Pantoprazole irreversibly binds to the H+/K+-ATPase enzyme (proton pump) in the gastric parietal cells, inhibiting the final step of acid production.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour, full effect within 2-3 days.
Excretion
Primarily excreted in urine (approx. 80%) and feces (approx. 18%) as metabolites.
Half life
Approximately 1 hour.
Absorption
Rapidly absorbed after oral administration, bioavailability is about 77%. Peak plasma concentrations within 2-2.5 hours.
Metabolism
Extensively metabolized in the liver via cytochrome P450 (CYP) system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to pantoprazole or other substituted benzimidazoles.
- Concomitant use with rilpivirine-containing products.

Warfarin
Increased INR/PT reported.
Clopidogrel
Possible reduced antiplatelet effect (clinical significance debated).
Methotrexate
Increased methotrexate levels.
