Description
Overview of the medicine
Aptofen 1 mg Syrup is used primarily for the prophylaxis of migraine and cluster headaches, and also as an appetite stimulant in underweight patients. It contains the active ingredient aptofen (Pizotifen-like action) which works by blocking certain natural substances (histamine and serotonin) in the body.
Uses & Indications
Dosage
Adults
Migraine prophylaxis: Initially 0.5 mg at night, gradually increasing to 0.5-1.5 mg daily in divided doses or as a single night dose. Max 3 mg/day. Appetite stimulation: 0.5 mg three times daily.
Elderly
Start with lower doses, e.g., 0.5 mg once daily, and adjust cautiously based on response and tolerability.
Renal_impairment
Use with caution. Dosage reduction may be necessary in severe impairment. No specific guidelines, monitor for adverse effects.
How to Take
Orally. Can be taken with or without food. To minimize sedation, initial dose can be taken at bedtime.
Mechanism of Action
Aptofen (Pizotifen-like) acts as an antagonist at serotonin (5-HT1 and 5-HT2) and histamine (H1) receptors. Its antiserotonergic action is thought to be responsible for its migraine prophylactic effect, while its H1 antagonism and possibly other central effects contribute to its appetite stimulating properties.
Pharmacokinetics
Onset
Migraine prophylaxis: several weeks for full effect; Appetite stimulation: within days.
Excretion
Mainly excreted in urine, largely as metabolites.
Half life
Approximately 23 hours.
Absorption
Well absorbed from the gastrointestinal tract, peak plasma concentrations reached within 5-8 hours.
Metabolism
Extensively metabolized in the liver, primarily by N-glucuronidation.
