Overview of the medicine
Aquacal-D 500 mg Tablet is a combination medicine containing Calcium Carbonate and Vitamin D3 (Cholecalciferol). It is used to supplement calcium and vitamin D in individuals who do not get enough from their diet or have specific conditions requiring increased intake, promoting bone health and preventing deficiency states.
Uses & Indications
Dosage
Adults
Usually 1-2 tablets daily, or as directed by the physician. Should be taken with meals.
Elderly
Same as adult dosage, but caution with renal impairment. Dosage adjustment may be required.
Renal_impairment
Use with caution and monitor calcium and phosphate levels. Dosage adjustment may be required.
How to Take
Orally. Tablets can be chewed or swallowed whole. Best taken with food to enhance absorption and reduce gastric upset. Do not take with high-fiber meals.
Mechanism of Action
Calcium Carbonate provides elemental calcium which is essential for bone formation, maintenance, and various physiological functions like nerve transmission and muscle contraction. Vitamin D3 (Cholecalciferol) is crucial for regulating calcium and phosphate homeostasis, facilitating the absorption of calcium from the gut, and its deposition into the bones, thereby increasing bone mineral density.
Pharmacokinetics
Onset
Effects on calcium levels can be seen within hours, but significant bone health benefits require chronic administration.
Excretion
Calcium is primarily excreted via feces, with a smaller amount via urine. Vitamin D3 metabolites are excreted mainly in bile and feces, with some renal excretion.
Half life
Elemental calcium has a variable half-life. Vitamin D3 has a half-life of about 15-20 days, but its active metabolite calcitriol has a shorter half-life.
Absorption
Calcium Carbonate is absorbed primarily in the small intestine, with absorption efficiency varying depending on dietary intake and vitamin D status. Vitamin D3 is well absorbed from the gastrointestinal tract, especially in the presence of bile salts.
Metabolism
Calcium is not metabolized. Vitamin D3 is hydroxylated in the liver to 25-hydroxyvitamin D (calcifediol) and then in the kidneys to the active form, 1,25-dihydroxyvitamin D (calcitriol).

Drug Interactions
Digoxin
High calcium levels can potentiate the effects of digoxin, increasing the risk of arrhythmias.
Bisphosphonates
Calcium can interfere with the absorption of bisphosphonates. Take calcium at least 30-60 minutes after bisphosphonates.
Thiazide diuretics
May increase the risk of hypercalcaemia by reducing urinary calcium excretion.
Tetracyclines and Quinolones
Calcium can reduce the absorption of tetracycline and quinolone antibiotics. Take calcium at least 2 hours before or 4-6 hours after these antibiotics.
Thyroid hormones (Levothyroxine)
Calcium can impair the absorption of thyroid hormones. Take calcium at least 4 hours apart from levothyroxine.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include hypercalcaemia (nausea, vomiting, constipation, abdominal pain, muscle weakness, polyuria, polydipsia, fatigue, confusion) and hypervitaminosis D. Treatment involves discontinuing the supplement, hydration, and managing severe hypercalcaemia with diuretics or corticosteroids.
Pregnancy & Lactation
Considered safe and often recommended for supplementation during pregnancy and lactation when dietary intake is insufficient or deficiency exists, under medical supervision. Excessive doses should be avoided.
