Description
Overview of the medicine
Ardance-M is a combination oral antidiabetic medicine containing Vildagliptin and Metformin HCl. It is used in adults with type 2 diabetes mellitus to improve glycemic control as an adjunct to diet and exercise.
Uses & Indications
Dosage
Adults
The recommended dose is one tablet of Ardance-M 50 mg/500 mg, 50 mg/850 mg, or 50 mg/1000 mg twice daily, with meals, depending on the patient's current metformin dose and individual response. Maximum daily dose of Vildagliptin is 100 mg and Metformin is 2000 mg.
Elderly
No dose adjustment is required based on age alone. However, renal function should be assessed before initiating treatment and regularly thereafter in elderly patients.
Renal_impairment
Not recommended for patients with eGFR < 30 mL/min. For eGFR 30-59 mL/min, the maximum daily dose of metformin should not exceed 1000 mg and vildagliptin should not exceed 50 mg. Close monitoring of renal function is necessary.
How to Take
Take Ardance-M orally, twice daily, with meals to reduce gastrointestinal side effects associated with metformin.
Mechanism of Action
Vildagliptin works by inhibiting dipeptidyl peptidase-4 (DPP-4) enzyme, which leads to increased levels of active incretin hormones (GLP-1 and GIP). These hormones enhance glucose-dependent insulin secretion and suppress glucagon secretion. Metformin, a biguanide, reduces hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization.

Onset
Glycemic effects typically begin within a few hours.
Excretion
Vildagliptin is primarily excreted via the kidneys (85%). Metformin is primarily excreted unchanged by the kidneys (90%).
Half life
Vildagliptin: approximately 3 hours. Metformin: approximately 6.2 hours.
Absorption
Vildagliptin is rapidly absorbed with an oral bioavailability of 85%. Metformin's absolute bioavailability is 50-60%. Food can affect absorption slightly.
Metabolism
Vildagliptin is primarily metabolized by hydrolysis. Metformin is excreted unchanged in the urine and does not undergo hepatic metabolism.
Side Effects
Contraindications
- •Hypersensitivity to Vildagliptin or Metformin HCl
- •Renal dysfunction (eGFR < 30 mL/min)
- •Acute or chronic metabolic acidosis, including diabetic ketoacidosis
- •Severe hepatic impairment
- •Acute heart failure or recent myocardial infarction
Drug Interactions
Alcohol
Alcohol potentiates the effect of metformin on lactate metabolism and can increase the risk of lactic acidosis.
Cationic Drugs
Cationic drugs that are eliminated by renal tubular secretion (e.g., cimetidine, amiloride, digoxin, morphine, procainamide, quinidine, ranitidine, triamterene, trimethoprim, vancomycin) may increase metformin concentrations by competing for common renal tubular transport systems.
Other Antidiabetics
May increase the risk of hypoglycemia if combined with insulin or sulfonylureas.
Iodinated Contrast Agents
Intravascular administration of iodinated contrast agents in metformin-treated patients can lead to an acute decrease in renal function and increase the risk of lactic acidosis. Discontinue Ardance-M at the time of or prior to the imaging procedure and withhold for 48 hours afterward, and restart only after renal function has been re-evaluated and found to be normal.
Storage
Store in a dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, discontinue Ardance-M and provide symptomatic and supportive treatment. Metformin overdose can lead to lactic acidosis, which requires immediate medical attention, including hemodialysis.
Pregnancy & Lactation
Limited data on use in pregnant women. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is unknown whether vildagliptin is excreted in human milk. Metformin is excreted in human milk. A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
