Description
Overview of the medicine
Aricarb 10 mg Injection is an antiemetic medication used to prevent and treat nausea and vomiting caused by chemotherapy, radiation therapy, or surgery. It acts on specific receptors in the brain and gut.
Uses & Indications
Dosage
Adults
10 mg administered intravenously (IV) slowly over 2-5 minutes, or intramuscularly (IM), typically 30 minutes before chemotherapy or surgery. May be repeated after 4-8 hours if necessary.
Elderly
No specific dose adjustment is required for elderly patients based on age alone, but caution is advised.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Aricarb 10 mg Injection should be administered intravenously (IV) over 2-5 minutes or intramuscularly (IM). For IV administration, it may be diluted in normal saline or 5% dextrose solution.
Mechanism of Action
Aricarb selectively antagonizes serotonin 5-HT3 receptors located both centrally (in the chemoreceptor trigger zone) and peripherally (in the vagal nerve terminals in the gut), thereby blocking the emetic reflex.
Pharmacokinetics
Onset
Within 30 minutes for IV, 1 hour for IM.
Excretion
Mainly excreted via urine, with a small portion in feces.
Half life
Approximately 4-6 hours.
Absorption
Rapidly absorbed following intramuscular (IM) or intravenous (IV) administration.
Metabolism
Primarily metabolized in the liver via cytochrome P450 enzymes.
Side Effects
Contraindications
- Hypersensitivity to the active substance or to any of the excipients.
- Concomitant use with apomorphine due to reported profound hypotension and loss of consciousness.
Drug Interactions
Tramadol
May reduce the analgesic effect of tramadol and potentially increase the risk of serotonin syndrome.
Apomorphine
Concomitant use may lead to severe hypotension and loss of consciousness.
Other serotonergic drugs (e.g., SSRIs, SNRIs)
Increased risk of serotonin syndrome.
Storage
Store below 30°C. Protect from light. Do not freeze. Keep out of reach of children.
Overdose
There is no specific antidote for Aricarb overdose. Management should be symptomatic and supportive, including monitoring vital signs and cardiac function (e.g., ECG).
Pregnancy & Lactation
Use during pregnancy is not recommended unless clearly necessary, as adequate human data are lacking. Caution should be exercised when administered to a nursing mother, as it is unknown if Aricarb is excreted in human milk.
