Description
Overview of the medicine
Arinet 50 mg Tablet contains Sertraline, a selective serotonin reuptake inhibitor (SSRI) used to treat major depressive disorder (MDD), obsessive-compulsive disorder (OCD), panic disorder, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD). It helps to restore the balance of a certain natural substance (serotonin) in the brain.
Uses & Indications
Dosage
Adults
Initial dose: 25-50 mg once daily. Maintenance dose: 50-200 mg once daily. Adjust dose gradually based on response and tolerability, typically in increments of 25-50 mg at intervals of at least one week.
Elderly
Similar to adult dosage, but use caution and consider starting at a lower dose (e.g., 25 mg daily) and titrate slowly.
Renal_impairment
No dosage adjustment is generally required for mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Arinet 50 mg Tablet should be taken orally once daily, with or without food. It is recommended to take it at the same time each day.
Mechanism of Action
Sertraline selectively inhibits the reuptake of serotonin (5-HT) by the presynaptic neuron, thereby increasing the concentration of serotonin in the synaptic cleft. This leads to enhanced serotonergic neurotransmission in the central nervous system.
Pharmacokinetics
Onset
Therapeutic effects may take 2-4 weeks to become apparent.
Excretion
Approximately 40-45% excreted in urine and 40-45% in feces, largely as metabolites.
Half life
Approximately 26 hours (range 22-36 hours) for parent drug; N-desmethylsertraline has a longer half-life (62-104 hours).
Absorption
Well absorbed after oral administration, peak plasma concentrations occur within 4.5-8.4 hours. Food slightly increases absorption.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19, CYP2B6, CYP2D6, CYP3A4, and CYP2C9, to N-desmethylsertraline, which is less active.
Side Effects
Contraindications
- Hypersensitivity to sertraline or any component of the formulation.
- Concomitant use with Monoamine Oxidase Inhibitors (MAOIs) or within 14 days of discontinuing an MAOI.
- Concomitant use with pimozide.
Drug Interactions
MAOIs
Risk of serotonin syndrome (hyperthermia, rigidity, myoclonus, autonomic instability, mental status changes).
Pimozide
Increased pimozide levels and cardiotoxicity.
Triptans
Increased risk of serotonin syndrome.
Warfarin
Increased prothrombin time and INR, leading to increased bleeding risk.
NSAIDs/Aspirin
Increased risk of bleeding.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, vomiting, tachycardia, tremor, agitation, dizziness, and rarely, seizures or coma. Management involves supportive care, maintaining a patent airway, monitoring cardiac and vital signs. Activated charcoal may be useful.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Small amounts of sertraline are excreted in breast milk; caution is advised when administered to a nursing woman.
