Description
Overview of the medicine
Arnac (Diclofenac Sodium) is a non-steroidal anti-inflammatory drug (NSAID) used for the relief of pain, inflammation, and fever. It is commonly prescribed for conditions such as arthritis, musculoskeletal injuries, and dysmenorrhea.
Uses & Indications
Dosage
Adults
50 mg, 2-3 times daily, preferably with or after food. Maximum recommended dose is 150 mg/day.
Elderly
Generally, the lowest effective dose should be used. Similar to adult dosage, but caution is advised due to increased risk of adverse effects.
Renal_impairment
Dosage adjustment may be necessary in patients with severe renal impairment. Careful monitoring of renal function is recommended.
How to Take
Take orally with a glass of water, preferably with or immediately after food to minimize gastrointestinal upset. Do not crush or chew sustained-release tablets.
Mechanism of Action
Diclofenac Sodium works by inhibiting cyclooxygenase (COX-1 and COX-2) enzymes, which are responsible for the synthesis of prostaglandins. Prostaglandins are mediators of pain, inflammation, and fever.
Pharmacokinetics
Onset
Onset of action for pain relief typically within 30-60 minutes.
Excretion
Eliminated predominantly through urine (approximately 60%) and bile/feces (approximately 35%).
Half life
Plasma elimination half-life is approximately 1-2 hours.
Absorption
Rapidly and almost completely absorbed after oral administration. Peak plasma concentrations are reached in 2-3 hours.
Metabolism
Extensively metabolized in the liver, primarily by hydroxylation and subsequent glucuronidation.

Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include lethargy, drowsiness, nausea, vomiting, epigastric pain, and gastrointestinal bleeding. Management involves symptomatic and supportive care. Gastric lavage or activated charcoal may be considered if ingested recently.
Pregnancy & Lactation
Pregnancy: Contraindicated in the third trimester due to potential premature closure of the fetal ductus arteriosus. Use with caution in the first and second trimesters only if the potential benefit justifies the potential risk to the fetus. Lactation: Diclofenac is excreted into breast milk in small amounts.
