Arnigen 24 mg tablet contains Betahistine, a medication primarily used to treat the symptoms of Meniere's disease, which include vertigo (dizziness), ringing in the ears (tinnitus), hearing loss, and nausea. It works by improving blood flow in the inner ear, reducing the build-up of pressure.
Uses & Indications
Dosage
Adults
Initially 8 mg to 16 mg three times daily, preferably with meals. Maintenance dose is usually 24-48 mg daily, divided into 2-3 doses. Arnigen 24 mg tablet is typically taken once or twice daily depending on physician's advice.
Elderly
No special dosage adjustment is required for elderly patients.
Renal_impairment
Caution is advised. No specific dosage recommendations are available; individual dose adjustment may be necessary.
How to Take
Orally, preferably with meals to minimize gastrointestinal discomfort. Swallow the tablet whole with water.
Mechanism of Action
Betahistine acts as a weak histamine H1 receptor agonist and a strong histamine H3 receptor antagonist. This dual action is believed to improve microcirculation in the inner ear by relaxing the precapillary sphincters, thus increasing blood flow to the stria vascularis. It may also facilitate histamine release from histaminergic neurons, leading to increased neuronal activity in the vestibular nuclei, which helps reduce vertigo symptoms.

Onset
Symptomatic improvement may be noticed within hours to days, but full therapeutic effect can take weeks or months.
Excretion
Primarily excreted in the urine as its main metabolite, 2-pyridylacetic acid, within 24 hours.
Half life
Approximately 3-4 hours (for its main metabolite, 2-pyridylacetic acid).
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract. Peak plasma concentrations are reached approximately 1 hour after oral administration.
Metabolism
Rapidly and extensively metabolized by oxidation into 2-pyridylacetic acid (main metabolite) and 8-hydroxybetahistine, which are pharmacologically inactive.
Side Effects
Contraindications
- •Known hypersensitivity to Betahistine or any component of the formulation
- •Phaeochromocytoma (a tumor of the adrenal gland)
Drug Interactions
Antihistamines
May reduce the efficacy of betahistine. Concomitant use should be avoided.
MAO inhibitors (e.g., phenelzine)
May increase the exposure to betahistine. Caution is advised.
Storage
Store in a cool, dry place below 30°C, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, abdominal pain, dizziness, somnolence, and seizures at very high doses. Treatment is symptomatic and supportive, with gastric lavage and activated charcoal if appropriate.
Pregnancy & Lactation
Pregnancy: There are no adequate data from the use of betahistine in pregnant women. Animal studies are insufficient. Not recommended during pregnancy unless clearly necessary. Lactation: It is unknown whether betahistine is excreted in human breast milk. A decision should be made whether to discontinue breastfeeding or to discontinue betahistine therapy, taking into account the benefit of breastfeeding for the child and the benefit of therapy for the mother.
