Description
Overview of the medicine
Arohaler Inhaler contains Salbutamol, a fast-acting bronchodilator used to relieve symptoms of asthma and chronic obstructive pulmonary disease (COPD). It helps to open up the airways in the lungs, making breathing easier.
Uses & Indications
Dosage
Adults
For acute symptoms: 1-2 puffs (100-200 mcg) as needed, up to 4 times a day. For prevention of exercise-induced bronchospasm: 2 puffs 15-30 minutes before exercise.
Elderly
Same as adult dose, adjust based on individual response and tolerability. Start with the lowest effective dose.
Renal_impairment
No dosage adjustment is typically required, but use with caution in severe renal impairment.
How to Take
Administer via oral inhalation. Shake the inhaler well before each use. Exhale fully, then seal lips around the mouthpiece. While actuating the canister, inhale slowly and deeply. Hold breath for 5-10 seconds. Wait at least 1 minute between puffs if more than one is needed.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors in the smooth muscle of the bronchial tree, leading to bronchodilation. This effect is achieved by increasing intracellular cyclic AMP (cAMP) levels, which relaxes the bronchial smooth muscle.
Pharmacokinetics
Onset
Bronchodilation typically occurs within 5 minutes of inhalation.
Excretion
Mainly via urine as metabolites and unchanged drug within 72 hours.
Half life
Plasma half-life of inhaled salbutamol is approximately 4-6 hours.
Absorption
Rapidly absorbed after oral inhalation, with peak plasma concentrations occurring within 2 hours. A significant portion is swallowed and absorbed from the gastrointestinal tract.
Metabolism
Primarily metabolized in the liver to an inactive sulfate conjugate.
Side Effects
Contraindications
- •Hypersensitivity to salbutamol or any excipients in the formulation.
- •Severe milk protein allergy (if the product contains lactose).
Drug Interactions
Diuretics (e.g., Furosemide)
May potentiate hypokalemia, especially with high doses of salbutamol.
Beta-blockers (e.g., Propranolol)
May reduce or block the bronchodilator effect of salbutamol and can cause severe bronchospasm.
Monoamine Oxidase Inhibitors (MAOIs) and Tricyclic Antidepressants (TCAs)
Increased risk of cardiovascular adverse effects (e.g., hypertension, tachycardia) due to enhanced sympathomimetic effect.
Storage
Store below 30°C. Protect from direct sunlight, heat, and frost. Do not freeze. Do not pierce or burn the canister, even when empty. Keep out of reach of children.
Overdose
Symptoms of overdose include tachycardia, tremors, palpitations, headache, dizziness, nervousness, and hypokalemia. In severe cases, cardiac arrest and death may occur. Treatment is supportive and symptomatic, with careful monitoring of cardiac status and potassium levels.
Pregnancy & Lactation
Pregnancy Category C. Use only if the potential benefit justifies the potential risk to the fetus. Salbutamol is excreted in breast milk; caution should be exercised when administered to a nursing mother. Consult a doctor before use.
