Overview of the medicine
Arokaast 10 mg Tablet contains Montelukast, a leukotriene receptor antagonist used for the treatment of asthma and allergic rhinitis. It helps to reduce inflammation and relax the airways.
Uses & Indications
Dosage
Adults
10 mg once daily in the evening.
Elderly
No dosage adjustment required.
Renal_impairment
No dosage adjustment required.
How to Take
Take orally, once daily in the evening, with or without food. For asthma, take in the evening. For allergic rhinitis, the timing may be individualized. For exercise-induced bronchoconstriction, take at least 2 hours before exercise.
Mechanism of Action
Montelukast selectively inhibits the cysteinyl leukotriene 1 (CysLT1) receptor. Leukotrienes are potent inflammatory mediators that are released from various cells, including mast cells and eosinophils, and cause bronchoconstriction, mucus secretion, and vascular permeability. By blocking these receptors, Montelukast reduces these inflammatory effects.
Pharmacokinetics
Onset
Within 1 day for asthma symptoms.
Excretion
Primarily excreted in feces via bile (approximately 86%), with less than 0.2% excreted in urine.
Half life
Approximately 2.7 to 5.5 hours.
Absorption
Rapidly and almost completely absorbed after oral administration. Peak plasma concentrations are achieved in approximately 2-4 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP3A4, CYP2C9, CYP2A6).

Contraindications
- Hypersensitivity to Montelukast or any component of the tablet
Drug Interactions
Rifampin
May decrease Montelukast plasma concentrations, possibly by inducing hepatic CYP3A4, 2C8, and 2C9.
Phenobarbital
May decrease Montelukast plasma concentrations.

Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose may include abdominal pain, somnolence, thirst, headache, vomiting, and psychomotor hyperactivity. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Use during pregnancy only if clearly needed and the potential benefit outweighs the potential risk to the fetus (Category B). Use with caution during lactation as Montelukast is excreted in breast milk.
