Description
Overview of the medicine
Asilee 500 mg tablet contains Asileemycin, a broad-spectrum macrolide antibiotic used to treat various bacterial infections, including respiratory tract infections, skin and soft tissue infections, and certain sexually transmitted diseases.
Uses & Indications
Dosage
Adults
For most infections: 500 mg once daily for 3 days, or 500 mg on day 1 followed by 250 mg on days 2-5. For Chlamydia: 1g single dose.
Elderly
No specific dose adjustment is required unless severe renal or hepatic impairment is present.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment (CrCl > 10 mL/min). Use with caution in severe impairment.
How to Take
Take orally with or without food. To minimize gastrointestinal upset, it can be taken with food. Do not chew or crush the tablet.
Mechanism of Action
Asileemycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit, thereby preventing bacterial growth and replication. It is typically bacteriostatic but can be bactericidal at high concentrations.
Pharmacokinetics
Onset
Within 2-3 hours of administration for clinical effect.
Excretion
Mainly excreted unchanged in the bile; a small amount is excreted in urine (about 6%).
Half life
Approximately 68 hours (terminal half-life), allowing for once-daily dosing.
Absorption
Well absorbed orally; bioavailability is about 37%. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Primarily metabolized in the liver, but extensive metabolism is not required for its activity. Active metabolites are minimal.
Side Effects
Contraindications
- •Hypersensitivity to Asileemycin or any other macrolide antibiotic
- •History of cholestatic jaundice/hepatic dysfunction associated with prior Asileemycin use
Drug Interactions
Digoxin
May increase digoxin levels; monitor digoxin levels.
Warfarin
May enhance the anticoagulant effect; monitor prothrombin time/INR closely.
Antacids (containing aluminum or magnesium)
Can decrease the absorption of Asileemycin; administer Asileemycin at least 2 hours before or 4 hours after antacids.
QT-prolonging drugs (e.g., Class IA and III antiarrhythmics, cisapride)
Increased risk of QT interval prolongation; co-administration is generally not recommended.
Storage
Store in a cool, dry place below 30°C, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management is supportive and symptomatic, with gastric lavage and general supportive measures.
Pregnancy & Lactation
Pregnancy Category B. Asileemycin should be used during pregnancy only if clearly needed. It is excreted in human milk; use with caution in nursing mothers.
