Description
Overview of the medicine
Asmain 120 mg Syrup is an anti-asthmatic medication used to treat and prevent symptoms of asthma and other obstructive airway diseases, helping to ease breathing.
Uses & Indications
Dosage
Adults
Initial dose: 10 mL (120 mg) two to three times daily. Dosage may be adjusted based on clinical response and serum Asmain levels, typically not exceeding 20 mL (240 mg) three times daily.
Elderly
Lower initial doses may be required due to reduced clearance and increased sensitivity; careful monitoring of serum levels is recommended.
Renal_impairment
Dosage adjustment may be necessary in patients with severe renal impairment due to altered clearance of metabolites. Monitor serum levels closely.
How to Take
Asmain syrup should be taken orally, preferably after meals to minimize gastrointestinal upset. Use a calibrated measuring cup or spoon provided with the syrup for accurate dosing. Do not use household spoons.
Mechanism of Action
Acts as a bronchodilator by relaxing the smooth muscles of the airways, leading to increased airflow to the lungs. It also has some anti-inflammatory effects.
Pharmacokinetics
Onset
Bronchodilatory effects usually begin within 30-60 minutes after oral administration.
Excretion
Mainly excreted by the kidneys, with approximately 10% as unchanged drug and the rest as inactive metabolites. Renal excretion is affected by urinary pH.
Half life
The elimination half-life varies significantly, typically ranging from 6-12 hours in adults, but can be longer in neonates, elderly, and those with liver or heart disease.
Absorption
Rapidly and completely absorbed from the gastrointestinal tract. Peak plasma concentrations are typically achieved within 1-2 hours after oral administration.
Metabolism
Primarily metabolized in the liver by cytochrome P450 enzymes (CYP1A2, CYP2E1, and CYP3A4) into active and inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to Asmain or any components of the formulation.
- Acute myocardial infarction.
- Severe uncontrolled hypertension.
- Untreated arrhythmias.
Drug Interactions
Beta-blockers
May antagonize the bronchodilator effects of Asmain. Concomitant use should be avoided or carefully monitored.
Phenytoin, Phenobarbital, Rifampicin
These drugs can induce the metabolism of Asmain, leading to decreased plasma concentrations and reduced effectiveness. Increased dosage of Asmain may be required.
Erythromycin, Ciprofloxacin, Cimetidine
These drugs can inhibit the metabolism of Asmain, leading to increased plasma concentrations and potential toxicity. Dosage adjustment of Asmain may be necessary.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach and sight of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, persistent headache, restlessness, tremors, tachycardia, cardiac arrhythmias, and seizures. In severe cases, cardiac arrest may occur. Management involves discontinuation of the drug, supportive care, and symptomatic treatment. Gastric lavage and activated charcoal may be considered to reduce absorption.
Pregnancy & Lactation
Asmain should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in breast milk; therefore, caution should be exercised when Asmain is administered to a nursing mother. Consult a doctor before use.

Lab Monitoring
- Regular monitoring of serum Asmain levels is crucial to ensure therapeutic concentration and avoid toxicity, especially during initiation or dosage adjustment.
- Liver and kidney function tests may be required in patients with pre-existing impairment or during long-term therapy.
Doctor Notes
- Educate patients on proper dosing and the importance of adhering to the prescribed regimen to optimize therapeutic outcomes and minimize side effects.
- Advise patients to report any significant adverse effects, especially cardiac or neurological symptoms, promptly.
- Consider serum Asmain level monitoring in patients with liver dysfunction, heart failure, or those on interacting medications.
Patient Guidelines
- Always take Asmain exactly as prescribed by your doctor. Do not alter the dose or stop the medication without consulting your doctor.
- Inform your doctor about all other medications, including over-the-counter drugs, herbal supplements, and vitamins, to avoid potential drug interactions.
- Seek immediate medical attention if you experience severe side effects such as chest pain, seizures, or severe breathing difficulties.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
Driving Precautions
