Description
Overview of the medicine
Asmalin 100 mcg Inhaler contains Salbutamol, a short-acting beta-2 agonist used to relieve symptoms of asthma and chronic obstructive pulmonary disease (COPD) such as wheezing, shortness of breath, and chest tightness. It works by relaxing the muscles in the airways, allowing air to flow more easily.
Uses & Indications
Dosage
Adults
For relief of acute bronchospasm: 1 or 2 puffs (100 or 200 mcg) as needed, up to 4 times daily. For prevention of exercise-induced bronchospasm: 2 puffs 15-30 minutes before exercise.
Elderly
No specific dose adjustment is usually required, but caution should be exercised in patients with cardiovascular comorbidities.
Renal_impairment
No dose adjustment is usually necessary for patients with renal impairment.
How to Take
Asmalin Inhaler is for oral inhalation only. Shake the inhaler well before each use. Exhale fully, then place the mouthpiece in your mouth. Press down on the canister while inhaling slowly and deeply through your mouth. Hold your breath for about 10 seconds, then exhale slowly. Use a spacer device as recommended by your doctor, especially for children or those with difficulty coordinating breathing and actuation.
Mechanism of Action
Salbutamol selectively stimulates beta-2 adrenergic receptors primarily in the bronchial smooth muscle, leading to relaxation of the airways and bronchodilation. This action helps to alleviate bronchospasm and improve airflow.
Pharmacokinetics
Onset
Bronchodilation begins within 5 minutes.
Excretion
Mainly excreted via the urine within 72 hours, both as unchanged drug (40%) and its metabolite (60%).
Half life
Approximately 2.7 to 5 hours.
Absorption
Well absorbed from the lungs following oral inhalation, with some systemic absorption from the gastrointestinal tract of the swallowed dose. Peak plasma concentrations are reached within 2-3 hours.
Metabolism
Primarily metabolized in the liver to an inactive sulfate conjugate (salbutamol 4'-O-sulfate).
Side Effects
Contraindications
- •Hypersensitivity to salbutamol or any component of the formulation.
- •Should not be used to stop premature labour when used with digoxin.
Drug Interactions
Diuretics
Increased risk of hypokalemia, especially when used with loop or thiazide diuretics. Monitor potassium levels.
Beta-blockers
Non-selective beta-blockers (e.g., propranolol) may antagonize the bronchodilatory effects of salbutamol and may cause severe bronchospasm in asthmatic patients. Concomitant use should generally be avoided.
Other Sympathomimetic Agents
Concomitant use with other sympathomimetic drugs may lead to additive cardiovascular effects. Use with caution.
Tricyclic Antidepressants (TCAs) and Monoamine Oxidase Inhibitors (MAOIs)
These agents may potentiate the cardiovascular effects of salbutamol. Use with caution.
Storage
Store below 30°C. Protect from direct sunlight and freezing. Do not puncture, burn or expose to temperatures over 50°C, even when empty. Keep out of reach of children.
Overdose
Symptoms of overdose include excessive beta-adrenergic stimulation such as tachycardia, palpitations, tremor, headache, dizziness, nausea, hypokalemia, hyperglycemia, and metabolic acidosis. Management is primarily supportive. Cardiac monitoring may be required. In severe cases, potassium levels should be monitored.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Salbutamol is excreted in human milk; use with caution in nursing mothers, only if the potential benefit outweighs the potential risk.
