Overview of the medicine
Asmaloc (Ketotifen) is an antihistamine and mast cell stabilizer used for the long-term prophylactic treatment of bronchial asthma, allergic bronchitis, and other allergic conditions. It helps prevent allergic reactions by stabilizing mast cells and blocking histamine receptors.
Uses & Indications
Dosage
Adults
Adults and children over 3 years: 5 mL (1 mg) twice daily with meals. May increase to 10 mL (2 mg) twice daily if needed. For optimal prophylactic effect, treatment should be continued for at least 2-3 months.
Elderly
No specific dosage adjustment required for elderly patients, generally follow adult dosage.
Renal_impairment
No specific dosage adjustment recommended for mild to moderate renal impairment. Use with caution in severe impairment, as metabolites are renally excreted.
How to Take
Take orally, preferably with meals to minimize gastrointestinal upset. It is a long-term prophylactic treatment and should not be used for acute asthma attacks.
Mechanism of Action
Ketotifen inhibits the release of inflammatory mediators (e.g., histamine, leukotrienes) from mast cells. It also acts as a non-competitive H1-antihistamine, blocking histamine's effects. Additionally, it may reduce eosinophil recruitment and activation, contributing to its anti-asthmatic effects.

Onset
Antihistaminic effects within 2-4 hours; prophylactic anti-asthmatic effects may take several weeks (4-10 weeks) to develop fully.
Excretion
Mainly excreted in urine (approximately 60-70% as metabolites) and feces.
Half life
Biphasic elimination: 3-5 hours (initial) and 21 hours (terminal).
Absorption
Well absorbed orally, approximately 60% bioavailability due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily via N-demethylation and glucuronidation.
Side Effects
Contraindications

Drug Interactions

Concomitant use may increase the risk of reversible thrombocytopenia.
Other antihistamines

Storage
Store in a cool, dry place, below 30°C. Protect from light. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, confusion, disorientation, nystagmus, convulsions, and respiratory depression. Treatment is supportive and symptomatic; gastric lavage or activated charcoal may be considered. Closely monitor vital signs.
Pregnancy & Lactation
Pregnancy Category C. Use only if clearly needed and the potential benefits outweigh the potential risks to the fetus. Not recommended during breastfeeding as Ketotifen is excreted in breast milk.
