Description
Overview of the medicine
Clopidogrel is an antiplatelet medication used to prevent blood clots in people with recent heart attack, stroke, or established peripheral arterial disease. It reduces the risk of further cardiovascular events.
Uses & Indications
Dosage
Adults
75 mg orally once daily. For ACS, a 300 mg loading dose may be given followed by 75 mg once daily.
Elderly
No dosage adjustment is generally required.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. Caution advised in severe impairment.
How to Take
Aspirel 75 mg Tablet should be taken orally, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Clopidogrel selectively inhibits the binding of adenosine diphosphate (ADP) to its platelet P2Y12 receptor, thereby preventing the activation of the glycoprotein IIb/IIIa complex and subsequent platelet aggregation.
Pharmacokinetics
Onset
Within 2 hours (antiplatelet effect). Maximal effect after 3-7 days of repeated dosing.
Excretion
Approximately 50% renally excreted and 46% fecally excreted within 120 hours after dosing.
Half life
Plasma half-life of clopidogrel is approximately 0.5-1 hour; active metabolite half-life is approximately 8 hours.
Absorption
Rapidly absorbed after oral administration. Prodrug, extensively metabolized.
Metabolism
Extensively metabolized in the liver, primarily by CYP450 enzymes (especially CYP2C19) to an active thiol metabolite and an inactive carboxylic acid derivative.
Side Effects
Contraindications
- •Hypersensitivity to the active substance or any component of the tablet.
- •Active pathological bleeding such as peptic ulcer or intracranial hemorrhage.
- •Severe hepatic impairment.
Drug Interactions
CYP2C19 inhibitors
May reduce the formation of the active metabolite of clopidogrel, leading to reduced efficacy. Examples include fluoxetine, fluvoxamine, ticlopidine, carbamazepine.
Warfarin and other oral anticoagulants
Increased risk of bleeding. Co-administration should be undertaken with caution.
NSAIDs (Non-Steroidal Anti-Inflammatory Drugs)
Increased risk of gastrointestinal bleeding. Use with caution.
Proton Pump Inhibitors (e.g., Omeprazole, Esomeprazole)
May reduce the antiplatelet effect of clopidogrel. Avoid concomitant use, especially in patients at high risk for cardiovascular events.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to increased bleeding time and potential bleeding complications. Management is primarily supportive. Platelet transfusion may be considered if rapid reversal of effects is required.
Pregnancy & Lactation
Clopidogrel should be used during pregnancy only if clearly needed and the potential benefits outweigh the potential risks to the fetus. It is unknown whether clopidogrel is excreted in human milk; caution should be exercised when administered to a nursing mother.
